- Observe, identify, and manage the hazards
- Eliminate deviations from current regulations
- Managed so that accidents can be prevented
immediately

(One way to solve a problem or assess a risk before an accident

occurs).

17
1. Inspection Preparation LOGO

A. Organizing inspections
- Create a schedule (Time, location/area)
- Prepare a trained inspector in your place
- Determine the duration of the inspection task
- Find a map/picture of the inspection location
- Make an appointment with the PIC Area
B. Review procedures/standards
- Prima Nirbhaya safety system
- Fatality Accident Prevention Elements
- Code of Practice
- Specifications and utilization technique guidelines

C. Ensure that the necessary equipment is available.

- Inspection checklist sheet
- Personal Protective Equipment
- Documentation Tool
- Facilities (vehicles, measuring instruments, etc.)

LOGO
Types of Inspection Based on the Process
Planned inspection
▪ Start with complete preparation, checklist, standard measuring
instruments, schedules, competencies and necessary
equipment

Unplanned inspection
▪ Walk around inspection and unsystematic
▪ Less focus on unsafe acts
▪ Unsafe conditions that require thorough attention are often
overlooked
▪ Recording is rarely conducted
▪ The improvement/prevention is inadequate.
22
Example of a Planned Inspection LOGO

▪ OSM – (On Spot Monitoring)

▪ PTO (Planned Task Observation)
▪ Prinasa – Superintendent Up Line Observation
▪ Prestart check Mobile Equipment (LV, Trucks)
▪ Fire Extinguisher Inspection
▪ Field supervisor daily inspection

23
On Spot Monitoring LOGO

Is a safety monitoring program that is focused on fulfilling

the safety standards and requirements of Prima Nirbhaya
KPC.
The Objectives of On Spot Monitoring
▪ to deal with “non-standard” conditions and/or practices
▪ Appreciate conditions and/or procedures observed in the
field that are "in accordance with standards."

Therefore, the process should not be limited by a specific check list

and/or the theme of the monitoring being performed.

24
2. Implementation of Inspections LOGO

▪ Use all of your five senses

▪ Use a checklist
▪ Check the whole area systematically
▪ Examine everything with the utmost care and precision.
▪ Look for non-standard practice procedures
▪ Use the standard as a guide
▪ Identify visible, hidden and evolving/developing hazards
▪ Discuss issues with the team
▪ Record the hazards and violations

25
Planned Task Observation (PTO) LOGO

Is a planned or scheduled physical observation made by a

supervisor of work activities to verify whether procedures are
made and followed correctly. This also includes the
implementation of corrective actions for deviations from safe work
practices identified during the PTO

PTO Purpose
▪ To ensure that the implementation of SOP is in accordance with
the safety requirements as stated in the procedure
▪ Reference :
✔ Standard Operating Procedures (SOP)
✔ Standard Work Instructions (SWI)
✔ Job Safety Analysis (JSA)

26
PRINASA (Prima Nirbhaya Safety Audit) LOGO

Is an audit/monitoring of employee
behavior at work which is performed by
the Superintendent Level and above

Objectives of Prinasa
▪ Observe safe behavior at work
▪ Encourage safe behavior in performing
work in the field

27
Hazards and Risks LOGO

▪ Hazard is something that has the

potential to harm people,
property, the environment and the
production process
Example: slippery and oily floor
▪ Risk is the meeting of two or
more hazards that can cause a
number of losses
▪ example: people walking on
slippery and oily floors

28
Hazards Divided into Three Main Categories LOGO

A. The hazard is visible and obvious.

Example: easy to see, for instance: lack of lighting,
cleanliness and tidiness etc.

B. Hidden / Concealed hazard.

Example: not very clear, for instance: toxic gases, vapors in a
closed room

C. Developing hazard.
Example: things that degrade over time, for instance:
corroded slings, warped foundations, chemical reactions,
rust, and cracked concrete foundations.

29
LOGO
What will we do if we come across HAZARDS?

Leadership Techniques ✔ STOP the work

✔ Perform two-way communication
✔ Ask for an explanation of what to do:
o Understanding of the rules
o The reasons for taking the action
o Ask the Risk
o Ask how to prevent accidents (Commitment so
accidents are not repeated)

Say :
"I believe If you consistently do it. This work location will always be safe”

35
3. Making Recommendations & Follow Up LOGO

Recommendations are made based on the findings:

✔ Risk category
✔ Impact that can occur
✔ Compliance with applicable regulations
✔ Based on the risk control hierarchy

36
Hazard Control Hierarchy Concept LOGO

❖ Elimination (removal )
Eliminate hazards or harmful work practices.
For example: cleaning up oil spills on the work floor

❖ Substitution (replacement)
Replace tools, methods, materials from high risk to low risk for the same purpose.
For example: Replacing chemical A with chemical B which is more environmentally friendly

❖ Engineering ( Design & Insulation )

Designing tools, roads according to engineering and operational engineering principles
Isolate by separating the source of hazard
For example: Designing a road intersection design, Genset is given generator housing and noise reduction

❖ Control (administrative)
This includes work practice policies, work guidelines (training, KIMPER, signs, spotters, work permits, etc.)

❖ Fifth option Use of personal protective equipment (PPE)

Use personal protective equipment to reduce the severity of exposure to hazards in the workplace

37
4. Making Reports & Disseminating Inspection ReportsLOGO

✔Reports are made based on the findings

✔Write down all of the findings (Positive & Negative)
✔Appreciation aims to provide psychological
encouragement to maintain commitment
✔Show real pictures
✔Write recommendations from existing findings (according
to the Hierarchy of Risk Control & Regulation, applicable
standards)
✔Distribution to related parties

5. Inspection evaluation LOGO

The evaluation aims to assess whether the existing inspection

program:
✔ Run consistently & Commitment
✔ Fulfillment of OHS management system (PDCA)
✔ Monitor the progress of the follow-up to completion
✔ Covering the risks that exist in the company (OHSE &
Regulations)
✔ No work accident, no more serious liability (Warning, the
impact of violation of the law) due to not being followed up
by the person in charge

43
5. Inspection Evaluation (Continued) LOGO

How to complete:
1. Create a monitoring database
2. Display follow-up fulfillment percentage
3. Give firmness/treatment if the inspection findings
are not completed

44
5. Inspection Evaluation (Continued) LOGO

How to act as an inspector in the work area to

monitor:
1. Make a list of findings
2. Ask the admin for periodic reminders to the
person in charge of inspection findings
3. If providing minutes/program agenda of facility
repairs (Use a receipt)

45

Example of Inspection Monitoring

▪ Dashboard Monitoring (Safety Inspection)

46
Inspection Advantages LOGO

1. Prevent work process disruptions that can result in accidents by:

✔ Immediate repair if there is Unsafe Condition & Unsafe Act
✔ Direct contact with workers
✔ Increasing OHS awareness
✔ Encouraging workers to respond to Unsafe Condition and
Unsafe Act
✔ Implementing the OHS program

2. Carry out the supervisory function

3. Preventing Losses (Tools, Processes, Personnel, Environment,

Lawsuits through proper inspection procedures)
