DEFNITION
• Gonioscopy refers to the technique
used for viewing the anterior
chamber angle of the eye for
evaluation ,management and
classification of normal and
abnormal angle structures.
METHODS OF GONIOSCOPY

• DIRECT GONIOSCOPY
• INDIRECT GONIOSCOPY
*with indentation
*without indentation
DIRECT GONIOLENS
• Koeppe goniolens
• Richardson-Shaffer
• Huskins barkan
• Sieback
• Swan Jacob
• layden
TECHNIQUE OF DIRECT GONIOSCOPY

1. Patient lies supine with examiner

sitting on the side ofthe eye to be
examined."
2. After topical anesthesia, goniolens
is positioned onthe cornea, using
balanced salt solution or
methylcellulose.
CONTINUATION…

• 3. Examiner holds the goniolense in

one hand and thelight source in
other and scans the anterior
chamberangle by shifting his or her
position until all 360degrees have
been studied.
TECNIQUE OF DIRECT
GONIOSCOPY
INDIRECT GONIOSCOPY
• The light rays are reflected by
mirror in the contact lens (gonio
prism)
• Provide a mirror image of the
opposite angle & used in
conjunction with a slit lamp
INDIRECT GONIOSCOPY
• 2 TYPES
1.Non indentation
2. indentation
NON INDENTATION
TECHNIQUE
After anesthetizing the cornea,
the coupling gel is inserted into
the cup of gonio lens.
Gonio lens is placed and tipped
onto the cornea
CONTINUATION…..

• Slit lamp beam is focused on the

mirror
Which shows the inverted angle
image.
NON INDENTATION
GONIOSCOPY
GONIO PRISMS (NON INDENTATION

Surface if slightly larger than cornea&

requires a coupling gel.
1.Goldmann single mirror
2.Gold mann-3 mirror
3.Allen-thorpe
GOLD MANN MIRRORS
ALLEN THORPE PRISM
INDENTATION GONIOSCOPY
• Varrying amount of pressure applied to
the cornea with a tear coupled indirect
contact lens.
• Examiner observes the effect on angle
width
• increased pressure indents the central
cornea & displaces fluid into the angle,
opening it wider.
GONIOPRISMS (INDENTATION)

• Zeiss four mirror

• Sussmman four mirror
• Tokel single mirror
• Posner four mirror
ZEISS FOUR MIRROR
SUSSMMAN FOUR MIRROR
TOKEL SINGLE MIRROR
POSNER FOUR MIRROR
INDICATION

• Suspected angle closure disease

• elevated iop
• Pigment dispersion syndrome
• History of any type of glaucoma
• Ocular blunt trauma or history of foreign body
• Retinal vascular occlusion
• History of ocular tumor
CONTRA INDICATION

• Recurrent corneal erosion

• corneal abrasion
• Patients with keratopathy(i.e bullous,
band,punctate)
• Perforating eye injuries
