Affidavit of Disinterested Person

I, ____________________, of legal age, Filipino, do hereby state under oath that: 1. I am not a party to the case, De Guzman v. Palmer Asia, a Labor case filed with the NLRC; 2. I have no vested legal, financial, or inchoate interest in the matter in therein or the outcome thereof; 3. I am not related to the parties thereof;

4. To the best of my knowledge, the total monthly gross income of Ronaldo De Guzman does not exceed an amount double the monthly minimum wage of an employe; 5. To the best of my knowledge, Ronaldo De Guzman does not own any real property that has a fair market value (as stated in the current tax declaration) of more than Three Hundred Thousand Pesos (PHP 300,000.00). I am executing this affidavit so that Ronaldo de Guzman may qualify as an indigent party who may be exempted from the payment of legal fees, as provided for in A.M. 04-2-04-SC dated Juy 20, 2004, which revised Rule 141 of the Rules of Court.
