Standard/Essential Learning: 5.NF.1: Add and subtract fractions with unlike denominators (including mixed
numbers) by replacing given fractions with equivalent fractions in such a way as to produce an equivalent sum
or difference of fractions with like denominators.
SMART Goal: By the end of T2, students will increase their pre-assessment score by 20% OR 80% OF
STUDENTS WILL ACHIEVE 80% PROFICENCY when adding and subtracting fractions with unlike
denominators by using equivalent fractions to produce an equivalent sum or difference with like denominators.
Plan: Using CFAs to pre-test and determine leveled groups, each teacher will instruct for 2 to 3 weeks then we
will post-assess. The groups will be fluid with students moving from teacher to teacher based on proficiency.

Reflections: Even though we had some students that didnt achieve proficiency, nearly all of them achieved the
goal of increasing their pre-assessment score by at least 20%. It is always good to see growth in our students
even if they dont reach proficiency, and using the PLC model is showing us how each of them is successful.
