Terms of CPR class:
Scheduling:
There must be a minimum of 4 people scheduled for the class or it will be canceled.
I must have two weeks notice of a schedule date (For euipment availabilit! reasons".
I ask that !ou please notif! me within 4# hours before the scheduled class$ if !ou need to
cancel or reschedule$ otherwise I will have alread! picked up and paid for euipment and
cards.
Payments:
Pa!ment is e%pected before or the da! of the scheduled class$ no e%ceptions.
If an emplo!ee does not show up for the class$ I e%pect pa!ment for this person
re&ardless ('ue to m! pa!in& for euipment and a card for this person up front".
I accept cash$ compan! check or credit card.
I will provide an invoice when a compan! pa!ment is made to me. ( receipt will be
available for an! individual pa!ments ()pon reuest".
Video equipment:
*ou will need to provide a T+ and '+' pla!er with a remote$ if !our class is an on site
trainin&. ,ith advanced notice$ I will be able to provide a T+ and-or '+' pla!er for a
./0 transport fee.
