Subject;Accident at Marina Way on today(08/02/2009)

We would like to make report for Accident which damaged the existing Sing-Tel cable along Changi Business
Park Crescent during diaphragm wall excavation work at Panel OS30A on early hour of this morning.
In the event of diaphragm wall excavation work and accident are mention as following ;

-30-April, 01:30~07:30hr Investigate near the OS30A
existing Singtel Cable line and carried out trial trench.
Pumping out the bentonite slurry inside the OS03A panel to expose
the damage cable level. Found that SingTel cable across the Panel 30
were damaged which may happened during 2nd bite excavation of
Panel OS030A.
-30-Aprl, 07:30hr to 09:30hr Further investigation was carried out to conclude root cause of
damage.
-30-April, 09:30hr to 10:00hr based on further investigation, found that 2nd bite was excavated
based on panel length 6000m instead of approved panel length
4.823m.Therefore excavation encroached into the OS030 panel and
hit existing cable that caused Two live existing cable damage.
