AFFIDAVIT RE: ONE & SAME PERSON

I, JOCELYN N. ALMERIA, of legal age, Filipino, married, and a
resident of Ubanon District, Catbalogan, Samar, after having been
first sworn according to law, hereby depose and state: THAT--1. I subscribed my daughter JINKY N. ALMERIA as a scholar of
CAP CFEP (ONE UNIT) for P9,280.00 under SFA Account No. 77-8327-03-00033 evidenced by a Certificate of Full Payment No. EO-00000037-6 issued by CAPs authorized representatives.
2. My daughter is ordinarily known as JINKY N. ALMERIA (sans
the name MA.) in the family as well as in her other papers.
3. However, it is only in the CAP subscription that the word
MA. Was added to her name, without the fault of herein affiant, the
subscriber herself of said educational plan of CAP.
4. That the scholar MA. JINKY N. ALMERIA appearing in said
CAP educational plan is one and the same person as my daughter
JINKY N. ALMERIA.
IN TRUTH WHEREOF, I have hereunto executed this affidavit
to attest to the truth of the foregoing facts for whatever legal purpose,
this 13th day of December 2007 at Tacloban City, Philippines.
