AUTHORIZATION AND CONSENT FOR RELEASE OF

INFORMATION

I,___________________________________ acknowledge, understand and agree that

Qatar Petroleum, and its authorized representatives (QP), must have the ability to
inquire into, and confirm where necessary, information I have given QP regarding my
past education and past work experience. QP understands that, for personal reasons, I
may not want to give my consent to QP to contact one or more of my previous
educational institutions or employers. In this case QP will respect my decision. However,
I acknowledge and understand that if QP does require such information, and I am
unwilling to give my consent to QP to confirm it, then the lack of my consent may have a
detrimental effect, and may even cancel, my employment prospects with QP.
Therefore, I hereby acknowledge and agree that Qatar Petroleum, in its sole discretion,
may priorize, and deem essential, information from one or all of the below sources as a
condition precedent for my employment with Qatar Petroleum.
I hereby release and save harmless Qatar Petroleum, and its authorized
representatives, from any and all claims or damages, of whatsoever nature, arising from
this Authorization and Consent of Release of Information.
