    To the Parents of:

    As you know, our class will be taking several field trips this year. We have planned trips to the fire
    station, the county museum, a local dairy, and an all-day trip to the zoo. For your child to participate in
    these trips, I will need the attached Consent Form signed and returned to me by September 24.

    The consent form also solicits volunteer chaperones. Please check the box if you can chaperone any of
    these trips.

    Thanks for your help, and we hope you will attend the next open house.
