INTERNSHIP AGREEMENT FORM

The academic curriculum of PDPM Indian Institute of Information Technology, Design & Manufacturing (IIITDM) Jabalpur focuses very strongly on hands on experience, interdisciplinary education and project oriented learning. Its agenda is to produce graduates who are not only technically competent but also possess other skills like capability to learn through experience, critical thinking, practical aptitude and ability to synthesize the solution. It also recognizes that not all aspects of learning can be taught in the conventional way of classroom (or laboratory) teaching methodology. Realizing that there are important elements of learning in an organization, the Institute has opened its academic programme for approximately six months long project-based internship (PBI) opportunity to its students to be executed after the completion of sixth semester. The internship aims to provide on-the-job experience or exposure to ongoing research and development in an organization under the supervision of able practitioners/researchers. The internship would contribute to the development of a student's comprehension on technical skills, knowledge and practical problems. 1. SSP Pvt. (IIITDM Jabalpur) Henceforth called Institute Regarding the internship between the aforesaid Organization and the student: Name: Syed Adeel Ahmed Henceforth, called Student This current convention is valid for the duration of the internship which will last from 10th May 2013 to 1st November 2013. 2. The main objective of this internship agreement is to ensure that Student is able to put into practice the training he received at the Institute. Furthermore, the internship must represent an added value experience for Student. The Employer must not profit directly from the presence of Student of the Institute in his Organization. 3. Tasks entrusted to Student during his internship may include one or more of the following : a. b. c. d. e. f. Hands-on training Real project-based assignments Research-based activities Team-work activities Leadership and management skills Safety awareness

The Organization must : a) Provide a suitable position for Student, preferably on an ongoing or futuristic turnkey/ research/ developmental project being carried out by the unit. b) Provide an immediate project supervisor who is willing to mentor Student on a relevant project and keep a track of the performance. c) Agree to allow on-site visit by Student Supervisor from the Institute. d) Guide Student in obtaining his/her internship objectives in continuous and uninterrupted manner. e) Complete the Students evaluation report at the conclusion of the internship. f) Provide a confidential assessment report on the work done by Student at the conclusion of her/his internship. The report should preferably be on units letterhead. 5. The period of internship would be of 25 weeks starting from May / June 2011 (but not later than first week of June) to the month of November 2011. Students who have completed their Semester VI will be eligible to undergo such internships. 5. During the internship, Student must comply with the Organizations regulations, these includes medical visits and Organization working hours. 6. In the event of any disciplinary problem, the Director of the Organization or the person responsible for the internship reserves the right to end this present convention after having informed the Institute administration. Before the Students departure, the

Director of the Organization or the person responsible for the internship must make sure that the warning addressed to the Institute administration has been received. 7. Students will not indulge in any illegal activities. 8. Student shall keep confidential all the information provided by the Organization and shall not divulge the same to any third person without prior written permission from the Organization 9. Considering the high level of Student, the Organization will pay the Student Rs.---per month along with the following perks: a) Food b) Accommodation 10. The Organization commits to give Student as well as Institute, a statement describing the nature, content and duration of the tasks. The Organization will send to the Institute a completed IIITDM Jabalpur Internship Assessment Form by email only at placement@iiitdmj.ac.in or by post at Placement Cell, PDPM IIIT DM, Jabalpur at the end of the internship. 11. At the end of the internship, Student would write an internship report. This report will be sent to Organization or to his/her External Supervisor and then, sent back to Institute, annotated. 12. The Organization, Student or Institute may terminate this contract with a prior notice of one week. 13. Signature
