The caregiver reports that the child is able to drink from a cup or a glass without
handle. While in the area, the examiner observed that the child can drink from her
glass without spilling much. She was very careful when handling the glass filled with
water.

 Removes Garment
The caregiver reports that the child can remove her clothes even when she’s not
told to. She states that the child can take off her shirt and her shorts without any
assistance.

The child was told to play with the examiner. The examiner handed the ball over
to the child. The child hesitates at first but in the end, throws the ball towards the
examiner.

 Scribbles spontaneously
The examiner got some paper and a pencil and placed it in front of the child. The
child suddenly picked up the paper and scribbled lines all over the paper.

 Tower of two cubes


The examiner put the wooden blocks in front of the child. The child suddenly
picked up the blocks and stacked them up as high as she can. She held them for a
long time until she’s sure that they’re steady, then let go of her hand. She was able to
stack up as high as five.

 Tower of four cubes


The examiner put the wooden blocks in front of the child. The child suddenly
picked up the blocks and stacked them up as high as she can. She held them for a
long time until she’s sure that they’re steady, then let go of her hand. She reached a

After three attempts, the child failed to name any pictures appropriately. The child
was only able to mimic the sounds made by the pictured animals.

The child was able to follow directions when the examiner told her to hand over
the blocks to her caregiver and to put down the blocks on the floor. However, when
the examiner told the child to put the blocks on the table, the child handed over the
blocks to her.

o Gross Motor

 Kicks ball forward


A ball was placed in front of the child. The examiner instructed her to kick the
ball. At first attempt, the child picks up the ball and throws it to her grandmother. She
hesitates the second time, but then kick the ball towards the examiner.

 Walks well
The examiner observed that the child can walk well without assistance or without
holding onto objects.

 Walks backward
The caregiver reports that the child is able to walk backward without falling. The
examiner observed that the child could walk backwards for three or more steps, then
stops.

 Walks up steps
The caregiver reports that the child can walk up the stairs of their neighbor (about
20 steps high), holding onto its railings.

 Stoops and recovers


The examiner placed the ball on the floor. The child picked up the ball without
falling out of her balance.
 Throws ball over hand
The examiner handed over the ball to the child. The examiner first showed the
child how the overhand throw was then instructed the child to throw the ball using an
overhand throw.

 Balance on one foot for one second


The child, after three attempts, failed to balance on one foot on either foot. She
was having a difficulty standing on only one foot.

 Jumps in place
The child, even without being told, was jumping all over the place.
INTERPRETATIONS

 PERSONAL – SOCIAL
The child is more interactive with other people although there is a slight anxiety
when meeting strangers. She shows independence as evidenced by using the spoon by
herself when eating, drinking from cup, and removing her garments by herself. She can
also imitate adults like doing housework. She can do simple tasks like running an errand.

 FINE MOTOR ADAPTIVE


She is able to scribble spontaneously and voluntarily. She attempts to imitate
letters. She can make a tower of up to 4 cubes. She can hold a spoon well but she still
has problem controlling her hand movements. She is able to open doors and small
bottles.

 LANGUAGE
The child is able to say 2-3 related words. She is still slurry when making sentences.
She sometimes uses nonverbal cues for the adults to understand her more.

 GROSS MOTOR
She can walk alone and can walk upstairs and seats in chairs she can reach without
assistance. She can run and jump. She can remove her garments without assistance. She
can recover from stooping.

 GENERAL INTERPRETATION
The child is of normal development in relation to her age. She has no delays in any of
the four sectors of development. Though there are two failed tests in two different
sectors, the child was able to pass all the other tests in her age line.
OTHER ASPECTS OF TODDLER ASSESSMENT

o NUTRITION
The child is on the weaning process. She can eat rice, bread, meat, fruits, and
vegetables, although her diet is composed mostly of milk. She is also fond of eating candies
and sweet food.

o PLAY
She is used to parallel play. She seldom shares her toys to her playmates but plays
willingly with adults. Recommended toys/ plays are blocks, and play-house or any play that
imitates adults (e.g. cooking/ other household chores)

o ELIMINATION PATTERNS
She voids not less than 5 times a day. Her usual color of urine is light yellow. She still
voids in bed. She defecates at least once a day in the morning. Her fecal consistency is moist
but formed, yellow to light brown in color.

o DENTAL HEALTH
She lost her two deciduous teeth (upper central incisors). She has dental caries on
almost all of her teeth. She merely brushes her teeth.

o REST AND SLEEP PATTERNS


She sleeps at an average of 10-12 hours. She sometimes takes a nap in the
