After thorough investigation and examination of all the evidences,
evaluations and reports submitted and on hand, including your letter of explanation dated September 30, 2016, you are found guilty of committing the following acts: 1. Falsifying the Official Receipt which forms part of liquidation. 2. Making it appear that the item ________ costs Php _______ when its fact its actual cost is ___________. 3. Misleading the management as to the actual costing of ____________, to the extent of defrauding the management. By reason of the above-enumerated infractions, you have violated the following provision of Globe Maritime Training Centers Code of Conduct: 3.3 AGAINST GMTC INTEREST AND POLICY 3.3.7 Any attempt to falsify or falsification of company records or documents and tampering with company equipment or facilities for the purpose of defrauding the company or to commit a dishonest act. Such violation cannot be condoned as it is inimical to company discipline and sets a bad example to others. Further, your letter of explanation failed to prove your innocence on the matter charged against you. As such, we regret to inform you that your employment with the company is hereby deemed terminated effective immediately upon receipt of this notice.
