This is in relation with the new policy regarding staff transfer of assignment as supported by operations
memo no.006, S-2012. This policy considers staff distance of assignment from residence. That, CARD
shall start considering assignment of not more than 100 km from his/her residence, but not in her/his
hometown. That, the institution will consider those who are assigned in far places shall be the first priority
to be transferred near the residence considering the number of years of service with the institution.
It is truly an honor and a privilege serving this Institution for my three years of stay now. I am blessed
enough that Im surrounded with great people here in Agusan Del Sur area for my three years of service
with CARD. However, going back and forth to my hometown requires a minimum of 6-7 hours travel
which sometimes hampers my desire to be with my family during weekends and special family gatherings.
With this, I am asking a consideration for transfer of assignment though I know that making this request is
a demand for the institution. It would be a pleasure if you will consider me to handle Carmen Unit, North
Cotabato B Area of Mindanao 7 region.
Rest assured that I will continue to strive for excellence in terms of my work and will continue to embrace
the core values of the Institution regardless of the challenges that I may experience in the future.
The approval of this letter request would be a great help for me to fulfill my other family responsibilities as
to taking care of my parents considering their ages now and as an older sister to discern the whereabouts of
my younger brother who is currently studying in Kabacan a few kilometers away from Carmen Unit.

This is in relation to Mr. Luis Salas case, a senior account officer of Barobo Unit, Surigao
Del Sur 1 area who was a victim of hold-up while on his way heading to the third center
meeting using Branch Motorycle with plate number LN 8933 in Barangay Rizal Barobo
Surigao Del Sur at around 9:45 oclock in the morning last March 26, 2012.
After a thorough validation and investigation, it was found out that Mr. Salas allowed
remitting member Editha Yungod (Center Durian) to travel with him using the Branch
Motorcyle after the latter requested a favor to get a ride in the motorcycle as to remit the
collection in the office since the third center is located Barangay Dapdap, passing Sitio
Kinayan which happens to be Mrs. Yungods residence- on the same route back to the
office.
Based on the report, Mr Salas while driving his motorcycle and approaching the
boundaries between Barangay Rizal and Kinayan, when they were approached by three
men wearing their bonnets. One of the three male suspects pointed a gun at them and
demanded anything they had. Mr Salas was robbed with his Samsung 3353 model costing
Php.4300 and a personal money of Php.200. In addition, center net collection amounting
to Php 5640 that was under the custody of Mrs Yungod was also taken during the
incident. The suspects then fled in an unknown direction on foot. Neither of the victims
was physically injured during the crime.
After a careful evaluation, it was found out that Mr. Salas has the negligence in ensuring
the Institutions safety and security upon allowing the remitting member to get a ride with
the Branch motorcycle which resulted to the incident.
With this, I am asking an approval to allow us to consider in charging the amount of Php
5640 as account receivables to Mr Salas as to avoid defaults for Center Durian.
Rest assured that all necessary investigation will be continued as to validate this case
alike.
