I, JULIAN JOZEL, single, of legal age, Filipino, and a resident
of 76-a Trading boulevard DAVAO CITY after being duly sworn to in
accordance with law, do hereby depose and state, that:
1. I am the mother of Ayeisha Jerika Malaran who
is 3 years old;
2. The father of my child, Ayeisha Jerika Malaran is
Louie James Malaran;
3. The father of my child, Louie James Malaran and I
are living together as husband and wife without
marriage for 3 years;
4. I am consenting, Louie James Malaran, the father
of my child to process my daughters passport in
my behalf with full authority;
5. I am executing this Affidavit in good faith, for
whatever legal purpose it may serve and to attest
to the veracity of the foregoing;
