Instructions for Completing Expense Report
1. Information can be entered in white cells only. Both blue and yellow cells are locked, and blue cells
are calculated cells.
2. Expenses are reported on a weekly basis, with Sunday as the first day of the week. Be sure the date
entered is a Sunday. After entering the Sunday date, other date fields will autopopulate, including
"period from" and "period to."
3. "Department" and "Product Line" have drop down menus. Make the appropriate selection and the
cell below will autopopulate with the corresponding number.
4. Common expense categories are listed; report expenses for these type items on the appropriate line.
5. Expenses that do not have a separate line item are reported on the miscellaneous line and must be
explained in the appropriate section at the top right of the form. Provide a brief description of the
expense so that accounting can code the expense appropriately. Phone and office supplies must be
briefly explained also.
6. Use the Notes/Coding Information section as needed to provide additional information about the
purpose of an expense or for other information you feel necessary or helpful to provide.
7. Breakfast, lunch and dinner lines are used to record meals eaten alone.
8. Business meals line is used when employees dine together, such as a working lunch. Provide details
in the section titled "Itemized Business Meals/Entertainment."
9. The entertainment line is to report expenses for meals that have a business purpose, such as lunch
for a doctor's office or a meal with a consultant. Provide details in the section titled "Itemized
Business Meals/Entertainment."
Deadline - Expense reports must be received by 5pm PST Monday for payment that week.
Those received after the deadline will be paid the following week.
Submit an expense report every week. If you have no expenses for a given week,
enter "No Expenses" in Purpose section under your name.
Mail original with receipts to your Regional Sales Manager for approval.
Home Office - Turn in approved expense report with receipts to accounts payable.
Payment Options
If you would like to sign up for ACH transfer, please complete the form "Direct Deposit, Expense
Reimbursement" located on the intranet and submit to Barbara Prall for processing.
ACH users - Important - Please notify Barbara Prall of changes to your account that would
affect your direct deposit so that your expense payments are not interrupted.
