This document lists the generally accepted therapeutic serum drug concentrations and toxic concentrations for several commonly used drugs. For each drug, it provides the conventional and SI units for the therapeutic range and any toxic levels. Some examples given are acetaminophen with a therapeutic range of 0.2-0.6 mg/dL and toxic over 5 mg/dL, amikacin with a peak therapeutic of 16-32 mcg/mL and trough below 8 mcg/mL, and vancomycin with a peak therapeutic of 30-40 mg/mL and trough of 5-10 mg/mL. Toxic levels are provided where applicable.
