AIA IHS CARD

• Once AIA IHS card is issued, this card MUST be

presented at any one of the listed panel GP.

• There will be no reimbursement if card is not

presented after card has been issued.
 TRADITIONAL CHINESE MEDICINE -
TCM VISIT

• Once AIA IHS card is issued, this card MUST be

presented at any one of the listed panel TCM.
Surcharge after normal operating hours will not be
covered.

• Reimbursement for consultation only.

• Six (6) visits per policy year.

 POLYCLINICS VISIT

• IHS card is not valid at Polyclinics, staff have to pay

in advance.

• Polyclinics visit will be reimbursed in FULL.

• Please submit original invoice and IHS Outpatient

claim form to HR for reimbursement.
 A&E IN SINGAPORE

• Max claim $150 each visit.

• Please submit original invoice and IHS Outpatient

claim form to HR for reimbursement.

In cases when an LOG is not available upon your hospitalization:

Attending doctor report (fee borne by member)

 Submit to HR - ORIGINAL FINAL INVOICE, together with Pre & Post

bills if any and discharge summary within 20 days of discharge.

*Note: You may still request for a LOG to be send to hospital even after your
admission if its an unforeseen hospitalization (not applicable to short stay)
