I, Kenneth John R. Mayo, student of Bachelor of Science in Industrial Engineering at Polytechnic

University of the Philippines – Santa Rosa Campus is requesting to please consider my enrolment
in POSC 1013 Politics and Governance with Philippine Constitution.

I requested to add the subject using an ACE Form this first semester 2017-2018, but unfortunately
it was not tag in my account. It was only lately that we found out that the name of my classmate
who did not enroll the subject was in the master list of our professor.

Hoping that you will consider my request because I am the one attending the subject only to find
out that my name was not in the master list.
