Academic

Requirements

A. For incoming freshmen


1. For Graduating Grade 12 applicants - First Semester Grades
2. For High School Graduates - Fourth Year Form 137;
B. For applicants with earned units in College
1. Certificate of grades in all subjects in completed semesters
C. For other applicants
1. ALS - Accreditation and Equivalency Test Passer Certificate
2. PEPT - Certificate of Advancing to the Next Level

Other Requirements

A. 1 pc. 1 x 1 Picture
B. Latest Income Tax Return (ITR) of parents or guardian
1. For those who are exempted from filing income tax, any of the
following should be submitted: a Certificate of Tax Exemption
from the Bureau of Internal Revenue (BIR), Certificate of
Indigency from their Barangay, Case Study from DSWD, or
Affidavit of No Income. For children of Overseas Filipino Workers
(OFW) and Seafarers, a latest copy of contract or proof of income
may be considered.
C. Certificate of Good Moral Character from the last school attended.
D. Solo-Parent ID or Certification from DSWD for applicants with single-
parent.
E. Applicants belonging to Ethnic Group - Certificate from National
Commission on Indigenous Peoples (NCIP)
