I, D​ominador C. Guinid. ​Of a legal age, married, Filipino and a resident of ​Poblacion,
Alfonso Lista, Ifugao,​ after having been duly sworn under oath in accordance with law,
Do hereby depose and state:
1. That I am the father and the legal guardian of ​RALEIGH DOMINIC C. GUINID, ​W
ho is graduating student of University of Cagayan Valley with principal address at
Balzain Tuguegarao City Philippines, taking up Bachelor of Science in Marine Engineeri
ng;
2. That one​ ​of the requirement for such graduation is to take one (1) year apprenticeship
training with any shipping company;
3. That my aforementioned son is applying for apprenticeship with the Aleson Shipping
Lines, Inc. in the city Zamboanga;
4. That being his mother, I hereby give my consent freely and voluntarily to board any
vessel of Aleson Shipping Lines, Inc. and by these reasons, I hereby wave my right to
recover any damage, which my son may suffer on account of his apprenticeship training
and even to the extent death.
5. That I will not hold the captain of the vessel of Aleson Shipping Lines, Inc.
Accountable for the damages and free the same from any civil or criminal liability arising
for from any untoward incident that may fall to my son in the course of this Apprentices
hip Training;
6. That I am executing this Waver out of my own free and voluntarily will and that I have
not been force, intimidated, threatened of influenced or influence by any person or perso
ns in executing the same.
