PARENT’S CONSENT

I, Editha A. Afable of legal age, Filipino, married and a resident of Funda, Hamtic,
Antique, Philippines, by these presents, hereby state and declare that:

I am the mother of Julie Izah A. Quijano a fourth year student of Bachelor of Science
in Accountancy. I would like to inform you that permission is hereby granted to my daughter to
participate in the training as a requirement in the course under the College of Business and
Accountancy and I understand that the said skills training will cover 400 hours.

I realize the risks connected with the said training but I believe that the experience will be
an essential part of his/her training. I am aware that the Practicum Coordinator will monitor the
performance of the student, and that the immediate superior within the company will take all the
necessary precautions to keep the student safe.

However, despite all efforts taken, untoward incidents beyond their control occur, we will
not hold CPU and the company where our son/daughter/relative took Student Internship Program
liable.

I attest to the truth of the foregoing facts.
