LETTER APPEAL

Sir:

Greetings

I am writing to request from your Good Office for the correction of my Date of Birth
on my PNP Identification card, Pay Slip, Income Tax Return and other pertinent
records.

My true and correct Date of Birth as recorded on my Birth Certificate is December

22, 1986 which is inconsistent with my Date of Birth appearing in my PNP
identification card, pay slip and income tax return which is December 22, 1996.
Attached with this letter is a copy of my Birth Certificate to prove that my Date of
Birth is December 22, 1986 and a copy of Affidavit of Discrepancy, duly notarized,
to explain the discrepancy of my Date of Birth in my Birth Certificate and my PNP
Identification Card.

I would also like to inform to this Good Office that from hereon, I will be using my
true and correct Date of Birth which is December 22, 1986 in all my legal papers,
records and other documents.
