““Preparation of Detailed project reports for Sewerage and Storm Water Drainage System in ULB’s of
Telengana” for the period of 6 months out of 6 months.

Andtherefore, certify that:

1. I have given my exclusive consent and authorisation to be included by EPTISA in its project, for the
position of Design Engineer- Sewerage water;

2. I am not aware of any situation of actual or potential conflict of interest in relation to the inclusion
of my CV in this specific project;

3. I will be available for the assignment as scheduled in EPTISA Technical Proposal, in accordance with
the Project requirement during the entire duration of the project.
