AFFIDAVIT OF EXPLANATION

I, AVE TINDUGAN TAPIA, of legal age, Filipino and resident of Blk 81 Lot 14, Victoria
Reyes, Dasmariñas City, Cavite, after being duly sworn to in accordance with law, depose and
state THAT

1. I am the daughter of SPS. MANUEL TESORERO TAPIA and LILY

TEVES TINDUGAN and was born on Sept 25, 1963 at San Miguel, Catanduanes.

2. That I used the name of AVE TAPIA RINT, my married name, in most of
my legal documents.

3. But ever since I had become a widow, I returned using the name of AVE
TINDUGAN TAPIA, my maiden name.

4. Through this affidavit I hereby declare under oath that the names AVE
TAPIA RINT and AVE TINDUGAN TAPIA, is my married and maiden name,
respectively, and the names herein is one and the same person.

5. I am executing this affidavit to attest to the truth and veracity of the

foregoing and for whatever legal purpose the same may serve;

IN WITNESS WHEREOF, we have hereunto set our hands this 8th day of March 2017 in
