AFFIDAVIT ONE AND THE SAME PERSON

I, MARIA SOCORRO ZITA R. REYES, of legal age, Filipino, married to

Renato S. Reyes, and with residence & postal address at No. 5 Sahara St.,
Better Living Subd., Paranaque City, after having been duly sworn to in
accordance with law hereby depose and say:

That I am a former PAL employee with PAL I.D. 223732 and I availed of
the Voluntary Separation Program in 1997;

That in my employee records, my name as appearing therein is MA.

SOCORRO R. REYES until my retirement. I found out lately that my
registered name according to my Birth Certificate is “MARIA SOCORRO
ZITA B. REYES” . My parents’ name are : ROMEO L. REYES and
ZORAIDA R. BAUTISTA-REYES. Copy of my said Birth Certificate is
attached as ANNEX “A”;

I am married to RENATO S. REYES, copy of my Marriage Certificate is

attached as ANNEX “B”;

That MARIA SOCORRO ZITA R. REYES and MA. SOCORRO R.

REYES as described above refers to one and the same person;

That I am executing this affidavit to attest to the truth of the foregoing for
all legal intents and purposes and to request PAL, SSS, BIR and other
offices to amend and correct my name to conform with my Birth Certificate
and other recoirds..
