1.0 PURPOSE:

The purpose of this standard is to define minimum requirements for work carried out at
height from structures that do not have guard rails or on cage lifted by a crane.

2.0 SCOPE

The standard covers Safety precautions for work carried out from a heights of 1.83
meters (6 feet) or greater, standing on a structure that does not have a guardrail and
precautions for lifting personnel on cages for work at height within CGC Sand it’s
Affiliate facilities.

3.0 DEFINITIONS

 Drop line: A line which extends from the anchorage point and to which a lanyard or
safety belt may be securely attached. The dropline should be at least 1.25 cm (1/2
inch) diameter nylon, 1.25 cm (1/2 inch) diameter polyester, 1.9 cm (3/4 inch)
diameter first-grade manila or 1.1 cm (3/8 inch) diameter steel cable with a
minimum breaking strength of 2455 kilos (400 lbs). The dropline shall meet the
NFPA 1983 Fire Service Life Harness and Hardware standard.

 Fixed Anchorage: A secure point of attachment, above head height, for droplines
and lanyards, capable of supporting a minimum static load of 2455 kilos (5400 lbs).

 Harness: A strap system designed to spread shock load over the shoulders, thighs
and seat area. It should be used where longer free falls (in excess of 2 meters) may
occur. The D-ring should be positioned on the upper back straps. The harness shall
meet the NFPA 1983 Fire Service Life Harness and Hardware standard.

 Lanyard: A flexible line, approximately 2 meters (6.5 feet) in length or less and at
least 1.25 cm (1/2 inch) diameter first-grade manila rope, to secure the harness to a
dropline, fixed anchorage or lifeline with a fall arrest mechanism. The lanyard shall
meet the NFPA 1983 Fire Service Life Harness and Hardware standard.

 Lifeline: An anchoring cable rigged between two fixed anchorage points to serve
as a mobile fixture points for the attachment of lanyards or droplines. The purpose
is to limit swing injuries by providing a continuous overhead fixture point as the
worker moves horizontally. Important factors to be considered include: an adequate
degree of sag, the shortest practical lengths possible, sufficient shock absorption
and design strength proportional to the number of workers who may use the line.

The lifeline shall meet the NFPA 1983 Fire Service Life Harness and Hardware
standard.

 Shall: Signifies mandatory requirements.

 Should: Signifies recommended/optional requirements.

4.0 REQUIREMENTS

CGC and its Affiliates shall develop procedures and programs that meets the following
requirements on fall protection for work carried out at height from structures that do not have
guard rails or on cage lifted by a crane:

CGC Company shall be consulted for any clarifications to this standard. The clarification given by
CGC comapny shall be complied with and considered final.

 Every effort shall be made to install scaffold/structures with guard rails to work at a
height of 1.83 meters (6 feet) or greater. However, if scaffolds/structure with
guardrails cannot be installed then Full body Safety harness attached to a permanent
structure shall be worn by employees working at height of more than 2 meters on a
platform or structure that does not have a guard rail.

 Full body Safety Harness/ lifelines and lanyard shall meet the NFPA 1983 Fire Service
Life Harness and Hardware standard. The use of a body/Safety belt for fall protection
is prohibited.

 All personal fall protective equipment should be inspected by the user prior to each
use. Harnesses, lanyards, droplines/lifelines, etc. which have been (or are suspected
to have been) subjected to shock or excessive impact loading SHALL BE DISCARDED.

 Lanyard and droplines/lifelines attached to the Safety harness shall be secured in a


manner to limit falls to less than 1.83 meters (6.5 feet) and must be capable of
supporting a load 2455 kilos (5400 lbs).

 The following requirements should be used for selecting fixed anchorage


attachment points:

A. 10 cm (4 inch) or larger piping (not aluminum), 10 cm (4 inch) or larger structural


members except elbows, tees and 15 cm (6 inch) or larger channels.

B. Insulated piping may be used if the piping size is known to be a minimum of 10


cm (4 inch) diameter and the attached surface is not too hot to the touch (nylon
lanyard and polypropylene drop line are sensitive to heat).
C. Extreme cold temperature service piping is not to be used as a fixed
anchorage.

D. Do Not Use: Pipes that are supported by other pipes or by pipe hangers,
aluminum piping, conduit, tubing, handrails, or angle iron.

 Special Requirements for Handling People at Height via Cages:

A. IT IS NOT RECOMMENDED TO USE CAGES TO LIFT PERSONNEL, UNLESS IT IS


IMPRACTICAL TO USE OTHER METHODS. THE USE OF ALL OTHER LIFTING
METHODS MUST HAVE BEEN CAREFULLY CONSIDERED.

B. CAGES SHALL NOT BE USED FOR LIFTING PERSONNEL UNLESS JOINTLY


APPROVED BY THE MAINTENANCE DEPARTMENT HEAD AND THE SAFETY
DEPARTMENT HEAD.

C. Cages and the slings shall have a third party valid load testing and certificate.

D. Cages should have a steel ring at the top for passing the sling.

E. Cages shall be fastened to the crane hook through sling.

F. Cage flooring should have a raised pattern with a 6 mm (1/4 inch) nominal
thickness.

 Cages shall have a top railing with a vertical height of 1 meter (3.3 feet) from upper
surface of top railing to the floor. It shall have intermediate railing halfway between
the top railing and the floor.

 All pipe railing shall be seal welded.

 Cages shall have a safety chain at the entrance and equipped with a proper latch type
hook at its moveable end. The chain shall not hang excessively and the maximum
deflection should not exceed .08 meters (3 inches).

 The safety chain at the cage entrance shall always be at the rear side of the person in
cage.

 Cage toe plates shall be minimum 8.9 cm (3 ½ inch) high, securely fastened and with
not more than 1.2 cm (1/2 inch) clearance above floor level.

 Cage working loads shall be assessed and marked on a plate attached permanently to
it.

 Annual inspection and load testing at 1.5 times the working load shall be carried out
and the date of testing should be marked on the load indicating plate.

 The sling used for lifting the cage shall be tested annually at 1.5 times the cage and
working load.

 Crane hook used for the cage shall have a safety latch.

 Full body Safety harness shall be used and it is to be tied to the crane hook through a
sling.

 All jobs to be performed while standing on the cage floor and cage railing shall not be
used for standing purpose.

 If the cage is to be raised more than 10 meters in height, two 1.2 cm (1/2 inch)
diameter ropes shall be tied to the cage on either side, in order to prevent the swing
action. These ropes shall be held on the ground by a person on each side.

 If the cage is to be raised up to a height of 10 meters only, one rope may be used.

 Wind direction and speed shall be considered before start of job. Hoisting shall be
stopped in case of high wind speed.

 Hoisting of personnel shall be performed in a slow, controlled and cautious manner.

 The area shall be barricaded or at least restricted for personnel not directly involved

Introduction

Falls from height continue to be the biggest killer on Construction Sites.

Common factors include: -

 Failure to recognize a problem.


 Failure to provide safe systems of work.
 Failure to ensure that safe systems of work are followed.
 Failure to use appropriate equipment.
 Failure to provide safe plant and equipment.
 Failure to provide adequate information.
 Failure to provide instruction and training.
 Failure to provide adequate supervision.

Fall Protection Rules


 Floor openings, gangways, elevated workplaces and other areas where persons could fall more
than 2.0 meters or over, must be protected with guard rails and toe boards.
 If suitable working platforms and guardrails cannot be installed, PPE (safety harnesses) must be
used and where possible the installation of a safety net.
 The use of PPE and safety netting must be considered as the ‘last resort’ in fall protection and not
the first option to be considered.
 Consideration must also be made to protect a person(s) from injury when the fall could be less
than 2.0 meters e.g. a fall from less than 2.0 meters but resulting in impalement on protruding re
bar or falling into some hazardous substances etc.
 All subcontractors to provide SECL with a copy of their ‘fall protection program’ before they
commence any work at height.

Guard rails  Main guardrail 910mm (minimum height).


& toe boards  Intermediate guardrails or other strong
barriers to be installed so that there is no gap
more than 470mm (prevents persons from falling underneath the
guardrail when working from a crouched position.)
 Toe boards at least 150mm high (prevents material from being
accidentally ‘kicked off’ the working platform and also, prevents persons
from sliding underneath the guard rails when slipping.)

Scaffold  Erected by qualified scaffolders.


 Inspected in accordance with the SECL HSE Procedure (SEM-1813).
 Do not use scaffold unless the ‘green scafftag’ is displayed.
 Alterations must not be made to the scaffold unless by authorized
scaffolders as per request by SECL supervision.

Mobile scaffold  Max height used inside must not exceed 3.5 times the minimum base
towers dimension.
 Max height used outside must not exceed 3.0 times the minimum base
dimension.
 Tower must not be moved when a person(s) is still on top.

Openings in floors,  Openings larger than 2 inches square or 2

Means of access  Means of access to elevated areas shall be approved type, of sound
construction, suitable for the purpose and used in a safe manner.
 No person(s) should have to climb a ladder longer than 9.0 meter.
 Vertical ladders longer than 9.0 meter must be provided with a platform
every 9.0 meters to enable a person(s) to take a rest.
 All ladders to be secured to prevent movement.
 All ladders to extend at least 1.0 meter above the stepping off and

stepping on point o provide a suitable handhold.


 Ladder openings in scaffold platforms to be kept to as small a size as
possible.
Lifelines & safety  Lifelines must not be ‘cosmetic’ they must be strong
nets enough to withstand a shock loading of a person
falling.
 Lifelines must be tested before use.
 Safety netting must also be tested before use.
 The netting must be installed as close to the
working area as possible to prevent person(s) from
falling a long way into the netting.

Safety harness  Safety harness is the only recognized fall protection


PPE.
 Safety belts must not be used.
 All person must be trained in the correct use of safety
harnesses especially attaching the life line to a
suitable fixing point.

Rescue  An emergency rescue plan must be in place to rescue any injured


person(s) working at height.
 The plan must include rescue of persons who have fallen and are left
suspended in their safety harness.

6.0 SAFETY AWARENESS

Working at heights accidents are usually serious and often result disabling injuries and
even death. Many of us assume that working at height injuries and fatalities occur as a
result of fall from height buildings and structures but they often occur from falls of only
two to four meters.

Portable Ladders

The most commonly used and abused equipment for working at


height is the portable ladder.

 Ladders should only used as a means of access from one height to another.

 Provide a three-points of contact is maintained with the ladder at all times.

 Ladders should be positioned on a firm.

 Level surface at an angle of 75 degrees.

 Always face the ladder when climbing up or down it.

 Keep your body centred within the ladder stiles

You should never position a


ladder:

 In front of outward opening doors.

 Against a window pane.

 On top of boxes, loose bricks or concrete blocks to gain extra height.

You should never:

 Use a ladder in high winds unless in an emergency.

 Stand any higher than 900 mm from the top of the ladder.

 Straddle the top rung

TOE BOARD

Scaffolding is a temporary work platform used for working at height. There are various
types of scaffolds that can be used but they all have some common safety
requirements.

 Guard rail between 38 inches and 45 inches.

 mid rail approximately midway between the Guard rail.

 A working platform which is at least 27 inches wide.

 Toe board around the edge of the platform ( to stop tools from falling on workers below)

 Scaffolding planks and toe boards that are secured and lashed at both ends.

 Access ladders that are securely lashed/fixed to the scaffold.

Never alter or modify any scaffold unless you are a certified scaffolder

A scaffold must be fitted with a tag at every access point to show:

RED/YELLOW/GREEN

 When the scaffold was erected and last inspected.

 The load rating of the scaffold

 When you are working off a scaffold you should always:

 Keep the walkways free of obstacles, tools and equipment.

 Keep scaffold platform free of grease and mud.

 Climb from one level to another using the provided ladders.

 Use a crane, hoist or winch to carry materials up to and down from the scaffold.

 You should never:

 Exceed the safe working load of a scaffold.

 Leave tools on railings or near the edge of the platform.

 Stand on the hand rail or mid-rail to reach the work.

 Work from a scaffold which has defective missing planks and rails.

Mobile tower scaffolds are used to do light-weight work and where there is a need to
frequently move the work platform from one point to another. When working from a mobile scaffold
make sure.

 It is no higher than three times the smallest base width.

 The caster brakes are locked on.

 You must be off the scaffold while it is moved

Fall arrest equipment should be worn whenever there is a risk of falling from
height. It must be used:

 When other means of protection, such as guard railing, is impractical.

 To provide additional protection.

The main reason for using fall protection equipment is to limit the
distance of any fall and minimise the risk of injury

A safety harness stops the fall and spreads the shock over a large area of the
body. This enables the fall to be stopped or arrested without causing bodily injury.
The safety harness must be attached to.

 A strap or belt lanyard which has a built-in shock absorber or self- retracting
lanyard which has a built-in inertia brake.
 The other end of the lanyard must be attached to a static line or
anchor point.

The length of the lanyard and the position of the anchor point determine the
amount of free-fall

Check the
harness
for any
signs of
wear or
damage to the fabric and stitching.

 Check the lanyard is firmly attached to the harness.

 Connect the lanyard to a static line or a secure anchor point.

 Make sure anchor point is positioned above where you are working and not below.

 If you are using strap or belt lanyard ensure there is a minimum of slack in the lanyard so
that you cannot fall any further than 1.8 meters.
