Second degree burn injury - Hot steam exposure

Incident
The Injured Person (IP) was performing boiler cleaning upon complete system depressurization,
drainage of liquid and isolation of valves. While the IP attempted to remove the bolts from the pipe
line flange of economizer return of the boiler for cleaning and de-clogging, he was exposed to the
released of hot steam residue from the clogged line which led to burn injury on his left hand and left
lower part of the abdomen.
He was immediately given first aid on board and referred to medical practitioner for further treatment.

Recommendations
Pending completion of the investigation, we strongly recommend the following measures to be taken;

1. Risk assessment related to high risk activities such as maintenance of pressure vessel, shall be
conducted and communicated to all personnel prior to performing the activities.
2. Works supervisor shall ensure that all potential hazards or energy from the equipment or
system have been fully eliminated and isolation completed prior to approval of Lock Out Tag
Out (LOTO) as per procedure requirements.

NO WORK IS SO URGENT THAT WE CANNOT TAKE THE TIME TO DO IT SAFELY
