the only agreement between __________________________________ [hereinafter
referred to as the ‘client’] and Hershey Sabino, HMUA.

Terms and Conditions: Wedding Package stated on the Quotation or Sales Invoice
Bookings: To secure a date, a signed contract is required with a 50% down payment
due at the time of signing. The down payment is non-refundable and non-
transferable. The down payment is deducted from the final balance due. Please be
advised that dates and scheduled makeup times will only be reserved with a signed
contract and when the down payment has been received.
Liability: All brushes and makeup products are kept sanitary. Makeup products used
are hypoallergenic. Any skin condition should be reported by the client to the makeup
artist prior to application and, if need be, a sample test of makeup may be performed
on the skin to test reaction.
Client(s) agree to release the makeup artist and her team for any skin complications
due to allergic reaction.
Booking Times: A start time and end time will be determined and approved by the
client. The makeup artist will arrive 30 minutes before the call time to set up. Each
makeup application will require at least 45 minutes to complete (at least 1 hour for
the bride). In the event that the makeup artist and her team are late, the makeup
artist will pay a fine of 100 Php per 10 minutes of tardiness.
In Case of Makeup Artist Cancellation: Inevitable events that are excusable such as
hospitalization and the like will disable the makeup artist to show up on the date of
the event. In the event that this happens, the makeup artist will give a replacement
in the same amount and quality agreed on.
In Case of Client Cancellation: Should a client cancel the booked event or any service
on the contract, the deposit will not be refunded or transferred.
