SUBJECT : Progress Report re: Death of PDL Felipe Villegas

1. Reference: Spot Report re: Death of PDL Felipe Villegas dated February 23,
2019

2. In connection with the above reference, the body PDL Felipe Villegas was
properly turned-over to Ms. Baby Villegas on February 23, 2019 at the
Oriental Mindoro Provincial Hospital (OMPH). The Death Certificate issued
by the City Civil Registrar states that the Immediate Cause of Death of PDL
Villegas is MULTIPLE CEREBROVASCULAR DISEASE INFARCT and the
Antecedent Cause is HYPERTENSION STAGE II. The time of death is 0232
A.M. and was pronounced dead by Dr. Ferdinand B. Ubanozo, Medical IV at
OMPH Hospital. The remains of PDL Villegas lies at St. Peter Chapel, Brgy.
Bayanan 1, Calapan City. Regional Trial Court Branch 39 where the case of
PDL Villegas is pending was already informed by Calapan City District Jail
through a letter.
