Dear Sir,
On date 6/8/05. a piece of rail was fed into the hopper by the pay loader breaking the stopper rail
grouted on the dumping point. Due to this at about 9.00 P.M Machine No. 37, BOCM CHP was broke down
damaging the following spares. The cost along with the parts is as follows.
Description of machine part Approximate cost.
1. Picks 08 Nos. @ Rs.450/- Rs. 3400.00
2. Pick holder 5 nos. @ Rs.800/- Rs. 4000.00
3.Crusher roll bearing 1 nos. @51000/- Rs.51000.00
4. Flight bar 2 nos. @3500/- Rs. 7000.00
5. Extended pin link 4 nos. @ 600/- Rs. 2400.00
Total Rs. 67800.00
In addition to this the wall plate of the said machine was dislocated which took two days to be
rectified departmentally. It is worthwhile to mention here that same type of damage has occurred on 28.6.05
and due to this the machine has to be stopped. It took 14 days (from 28.6.05 to 10.7.05) to repair the
machine and made O.K. for functioning.

Sub: - Damage of CHP on Dated 17.5.06
Dear Sir,
A buffer fabricated by welding two nos. of girders parallely of size .5 Mtrs X 4 Mtrs
was fixed on ground to check the tippers unloading coal from falling in to the hopper at CHP
of BOCM. On dated 17.5.06 at about 9.00 pm, this dumping point buffer was up-rooted and
thrown in to the machine hopper by the pay-loder of M/S RCP. Due to which the following
damages occurred.
1. Damage of the coal tipping position of feeder crusher N0-19
2. Damage of the side plate of hopper
3. i) Damage of 14 nos. of pick holders
ii) Damage of 11 picks
4. i) Damage of 3 nos. of flight bars
ii) Damage of 4 nos. of extended pin links
iii) Damage of 4 nos. of pin link of conveyer chain
The cost of the above damage is approximately Rs.40, 000.00
Moreover, on the same day in the 3rd shift at about 2.50 AM a piece of iron plate
measuring about 4.5’ X 10” size was dumped into the feeder crusher No-31 along with coal.
The piece of iron plate jammed at the delivery point of the machine. As a result the conveyer
belt of ‘A’ circuit was torn (0.25 m X 250 m) out of the full length of 350 m long belt. The
cost of the damaged caused is approximately Rs.3,00,000.00. Besides, the replacement of
the damaged portion of the belt with a new one was completed at 12.30 PM on dates 22.5.06,
causing a loss of 106 working hours.

Nos of crusher rolls were given to you for repair and fixing of new picks and holder. Also
vide Supply order No MCL/GM/LKPA/PUR/B-09/REV/2007-08/010 & 009 dated 15.5.07
you have supplied two Nos of Plummer block assembly. The same was fitted on the crusher
roll (of feeder breaker No 19) that you had repaired. The crusher roll was fitted for operation
on 17.12.07. The Inner racer of the bearings inside the Plummer block assembly cracked on
8.2.08. We had to replace both the bearings with new bearings. Even the shaft got worn out
at bearing seating.
You are requested to kindly replace the bearing and get the Shaft repaired at the
earliest.
