JOB ORDER/AGREEMENT

This Agreement is made between (name of technician) of (address of technician) and the client, (name of
Max) of (address of Max outside country), presently residing at (address of Max here in Cebu) for the
following:

Job Order : (detail of work to be done to his motor)

For : (description of his motor described in his motor CR)
Motor Owner : (name of Max)
Package Cost : Php (total amount), Labor and Materials
Date Paid : (date of full payment)
Received by : (name of technician)
Date due : (target date to finish)

That this Agreement should compromised the said technician to finish/turn over the said unit to the owner
with complete work done as per Job Order as full payment was already been made in advance;

That the Technician, should give updates to the motor owner every other day from the start of work for
its progress for the owner’s information status by means of communication or whatsoever ways;

That the Technician should provide the motor owner all the necessary receipts of purchased
parts/accessories for the unit for accountability;

That the Technician should comply the work and turn over the said unit, as to the Date Due specified to
the motor owner;

That any replacements of parts or revision, the Technician should inform the motor owner for conformity
before doing so;

That in completion of the job, the Technician should inform the motor owner for the latter’s acceptance
and conformity.

For and any acts done beyond the said Job Order description would resort to legal action.

In Witness Whereof, both parties affixed their signatures to affirm this Agreement with both Identification
Cards attached hereto.
