Peer Evaluation General Instruction: (1 whole sheet of Bond Paper)

1. Use the format given below.

2. Indicate your group number and project company name
3. Write the name of the members, do not include yourself. You will evaluate all the other members in your group.
4. Under Cooperation, you are to evaluate the other members in terms of their cooperation indirectly related to the project itself. (example: moral support,
participation, pakikisama, attendance, easy to deal with, financial support, etc.)
5. Under Content, you are to evaluate the other members in terms of their contribution directly to the project (example: assigned task, chapter 1, chapter 2,
etc)
6. For criterias, project and revision is 10 points each and presentation is 5 points. Ratings can range from 1 as the lowest to 10 as the highest for project and
revision. Ratings can range from 1 as the lowest to 5 as the highest for presentation.
7. Sum up all the scores per members under the Total column.
8. Write comments as needed to justify your ratings.
