2. CAGMANIPIS BRIDGE
- Nonstandard advance warning signs
- Incorrect location of signs
- No transition area
- No termination area
- Stockpiled materials were noted not confined
- No PPE’s

3. PISONG BRIDGE, MALAJOG BRIDGE, BULONGTO BRIDGE

- Provide traffic/flagman
- Have designated first aider and safety officer. Provide documentation (
logbook,minutes of meeting, reports etc.)
- Provide sufficient PPE’s
- Required the concerned Chief, Planning and Design Section to explain in writing
why:
a. No TMP was prepared for project in violation to DPWH ROADWORKS SAFETY
MANUAL 2004 Ed. D.O NO.13 series 2018
b. Programmed signages based on the approved DUPA/plans/specification are
made from nonstandard materials and not complaint of D.O NO. 13, Series of
2018
- Correct placement of all temporary signs and traffic control devices D.O No.13 2018
- Provide retro-reflective temporary traffic controls and devices
- Provide Speed restriction sign, “ROADWORK AHEAD” sign, “WORKMEN AHEAD” sign,
“PREPARE TO STOP” sign
- Provide temporary bollards 3 to 5 meters apart with barricade flasher light
- Provide “ END ROADWORKS” signs
