MEMORANDUM OF AGREEMENT

KNOWN ALL BY MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT is made and entered into by and between:

The EASTERN SAMAR STATE UNIVERSITY (ESSU), a chartered comprehensive


state university created by virtue of Republic Act 9312 under the Philippine laws,
represented herein by its President, Dr. EDMUNDO A. CAMPOTO, herein after referred to
as the ESSU.

-and-

The Land Bank of the Philippines with an office address located at Araba Building, San
Pedro Street, Borongan City, Eastern Samar represented in this agreement by
Christopher R. Marco.

WITNESSETH
WHEREAS, the Eastern Samar State University (ESSU), an institution of
higher learning located at National Highway, Maypangdan, Borongan City, Eastern
Samar, is offering On-The–Job Training in all its baccalaureate programs;

WHEREAS, the purpose of On-The-Job Training is to provide students with


important practical experience which complements their field of study and at the same time
will be of benefit to the industry of firm;

WHEREAS, the student internship program of the University is anchored on the


provisions of Commission on Higher Education (CHED) Memorandum Order No. 23 s.
2009;

WHEREAS, the university must select a HOST TRAINING ESTABLISHMENT (HTE)


and ensure the acceptability of practicum venue in order to protect student’s interest;

WHEREAS, the Land Bank of the Philippines hereby chosen as the Host Training
Establishment (HTE) and ensure acceptability of practicum venue in order to protect
student’s interest;

NOW, THEREFORE, for and in consideration of the premises and mutual covenants
herein set forth, both parties hereby agree to forge collaboration and partnership in pursuing
the implementation of internship activities to be performed both by ESSU and Land Bank of
the Philippines.

ARTICLE I

SCOPE COVERAGE AND POLICY STATEMENTS


That the scope of this agreement covers both the concerns of ESSU and Land Bank of the
Philippines.

1.1 As implementing partners for the Students Internship Program of Eastern Samar State
University.

1.2 That the implementation of all activities with the respect to the Student Internship
Program of ESSU shall be consistent with the CHED Memorandum Order, HTE’s
Policies and Guidelines and other applicable laws and policies, and

1.3 That the parties involved in this agreement affirm the need for a collaboration and
partnership in order to meet the objectives and goals of both parties.

ARTICLE II
RESPONSIBILITIES OF PARTIES
2.1 The ESSU shall;

2.1.1 Designate a coordinator to collaborate and supervise OJT practitioners during


the duration of the student’s internship.

2.1.2 Communicate with the Host Training Establishment (HTE) requesting


accommodation to undertake On-The-Job Training of students of this
University.

2.1.3 Brief the students on how they conduct themselves and their duties and
responsibilities before they field to the HTE.

2.1.4 Deploy the students to the cooperating agency with their needed
requirements.

2.1.5 Periodically check in person or by telephone with the agency head/supervisor


on the student’s progress.

2.1.6 Monitor and evaluate the performance of the student’s trainees jointly with the
Host Training Establishment (HTE).

2.1.7 Require the students to submit reports to attest the completion of their number
of training hours.

2.2 The Eastern Samar State University shall;

2.2.1 Facilitate the processing of documents of the students trainee/intern in


coordination with ESSU, College of Business Management and Accountancy
(CBMA).

2.2.2 Assign a permanent employee to provide clear direction and timely feedback to
the student trainee/intern. This individual will be considered as a worksite
supervisor for the students and is responsible for providing guidance, direction
and constructive criticism.

2.2.3 Give orientation to the trainees on HTE’s objectives, personnel, physical plant,
policies, rules and regulations.

2.2.4 Provide a safe workplace and advise student interns of any unforeseen
dangers associated with the workplace.

2.2.5 Develop intern job description outlining primary responsibilities of the students.

2.2.6 Assign, monitor and evaluate the tasks and projects. Following completion of
the internship, the supervisors completes a training evaluation and submits
directly to the OJT Coordination/Adviser.

2.2.7 Maintain communication with the University for Periodic Feedback as required
by the specific College, as well as the event of any special problem.

2.2.8 Certify the completion of the number of hours require for the semester.

2.2.9 Give recommendation and suggestions for the improvement of the University’s
On-The-Job Training Program.

This agreement shall take effect immediately upon the execution of this instrument
and shall enforced unless modified and terminated upon the consent of both parties.

IN WITNESS WHEREOF, the parties hereunder affix their signatures this June 14,
2019 at Eastern Samar State University, Borongan City, Eastern Samar, Philippines.
