1. The undersigned was granted motion for reconsideration/restoration on October 23, 2018
and reported to National Capital Region Police Office – Regional Personnel Holding and
Accounting Section (NCRPO-RPHAS) the following day October 24, 2018;

2. That, while the undersigned was still in the process of his papers for the said
restoration/activation of ATM & CFC’s, for unknown reason a reassignment order was
issued on February 11, 2019 and was received by the same on February 18, 2019;

3. That, due to unexpected circumstances worsen by financial and health condition, the
undersigned were only able to report on March 4, 2019;

4. That, the undersigned is scheduled to returned to PNP-Medical Health Service NCRPO on

February 7, 2019 re-possible kidney problem; (attached hereto are photocopies of Medical
Certificate dated February 18, 2019 validated by Police Major GLENN DALE B GARALDE
M.D. Medical Officer NCRPO)

5. That, due to said health condition and Doctor’s appointment, the undersigned already book
his return flight on March 7, 2019 at 1:40PM; (attached hereto are photocopies of said
scheduled flight)

6. That, the undersigned promise to return immediately as soon as he is cleared by the PNP
Doctor and to be able to get his issued ATM as well as pertinent documents needed for the
201 Police File as required;

7. That, the undersigned most respectfully prayed that his request be given utmost
consideration.
