The Sta. Cruz High School clinic is a health care facility within school intended to provide basic health
care for students and/ or refer them to the specialist/ primary health center if required.
CLINIC DESIGN
The School clinic comprise the following: Doctor room/ clinic has the adequate size (at least 14
square meters) to accommodate standard furniture, fixtures, medical equipment and supplies with
sufficient ventilation and lighting. Washbasin and taps water are available in the clinic. Treatment is not
less than 6 square meters, depending on school population and number of students. An additional area
for sick student’s observation is made available. The clinic has also a separate designated waiting area to
accommodate at least five students. Corridors and doors are wide and permits wheelchair and trolleys.
Walls are painted with easily washable paints with no sharp edges. The Clinic floors are made of easily
washable material like ceramic tiles or special medical floors. There are also a cabinet/ drawer to secure
files area for maintaining students records.
Only minimum amount of medicines are stored since the school only have one school nurse. School
nurse’s instructions were followed on the storage and delivery procedures of medications regarding
expire date, temperature control and pother pharmaceutical issues.
INFECTION CONTROL
Special attention is given to implement a comprehensive and up to date infection control standards.
Procedures are implemented to minimize the sources and transmission of infections and maintain a
sanitary environment. A system is in place to identify, manage, handle, transport, treat, and dispose of
hazardous materials and wastes whether solid, liquid, or gas. There is an active program for the
prevention, control, and investigation of infections and communicable diseases. Vaccine- preventable
diseases are always reported immediately and appropriate action taken to ensure the protection of
other children and adults in the school
Deworming at Sta. Cruz High School was held on August 7, 2018. Mrs. Jasmin F. Tacsiat and our our
Nurse Sherwin Joaquin gave a deworming tablet albendazole. As DepEd and DOH promotes a good
health of all students in both private and public schools.
Objectives:
Over 400 million school age children worldwide are infected with parasitic worms. These infections
harm their health and development and limit their ability to both access and benefit fully from their
educational system. There is a safe simple and cost effective solution: school- based deworming. Worms
affect both health and education of children. It potentially causing anemia, malnourished and
impairment of mental and physical development.
The school head and teachers assisted by the health personnel are expected to take the lead to
ensure success in the implementation of the program. The Department of Education continues its drive
to eliminate intestinal parasitism among public school learners nationwide through its mass deworming
program.
NARRATIVE REPORT ON SCHOOL BASED IMMUNIZATION PROGRAM MEASLES RUBELLA (MR) AND
TETANUS DIPTHERIA (TD)
The Department of Education (DepEd) supports the school based immunization program of
Department of Health (DOH) as the government response to ensure prevention of mobility and
mortality of school age children due to vaccine preventable disease.
This program aims to immunize all school children and adolescence enrolled in 1, 4, and grade 7 with
DOH recommended appropriate vaccines. Measles Rubella (MR) and Tetanus Diptheria (TD).
Last November 14, 2018, the School Based Immunization program was given by DOH at Sta. Cruz High
School to promote good health of the student and prevent this disease. Regional directors (RDs), school
division superintendents (SDs) and other school officials are enjoined to support in conducting this
activity. Health and nutrition personnel are also expected to coordinate with DOH regional and
provincial health offices for the implementation of the school- based vaccination program.
I. SUMMARY:
A health assessment is a plan of care that identifies the specific needs of a person and how those
needs will be addressed by the healthcare system or skilled nursing facility. Health assessment is the
evaluation of the health status by performing a physical exam after taking a health history.
This year Sta. Cruz High School conducted health assessment last July 12, 2018 with Mr. Sherwin
Joaquin our school nurse and with the coordination of Health Coordinator Ma’am Jasmin F. Tacsiat.
The objective of this assessment is by performing health assessment and physical examination the
nurse identify health patterns and evaluate each patient’s response to treatment and therapies,
using a comprehensive or focused approach, depending on the physical test situation considers
evidence- based recommendations for care based on PT’s values, the health provider’s clinical
expertise, or own personal experience. And patient- centered care plan, identifying the nursing
diagnoses, desired patient outcomes, and nursing intervention.
III. RECOMMENDATION:
We highly suggest to provide health awareness seminar with our parents, students and teachers
that will help each and every one to promote healthy living. Implement Feeding Program to the selected
student. Monitoring of student nutritional status by means of Body Mass Index (BMI). Planting fruits and
vegetable in the school and in the backyard. Maintaining cleanliness in the surroundings and have a
proper personal hygiene kit.
