SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

name, constitute and appoint __________________, with office address at
________________, to be my true and lawful attorney-in-fact and, in my name, place
and stead, to do and perform any and all of the following acts:

(1) To represent me in the ________________;

(2) To voluntarily appear on my behalf in any proceeding required in the case,

and to file and sign the appropriate responsive pleadings, oppositions,
motions, verifications, including a motion for reconsideration and appeal;

(3) To represent me in any case which might be filed in any court, tribunal or
administrative agency in connection with the case; to represent me in the
preliminary conferences, pre-trials, mediation and other proceedings; and
to appear and represent me in the hearing of the said case;

(4) To consider the simplification of issues in said case; to consider the

possibility of entering into stipulation of facts, and such other purposes of
pre-trial as provided under the Rules of Court; and to enter into
settlement or compromise agreements regarding the said case, with such
binding force and effect and under such terms and conditions as are
acceptable to them as if the same has complied with all the requirements
of Section 23 of Rule 138 of the Rules of Court;
