support me for my visit UK to attend 3rd International conference on Advance Dental Education
November 15 – 16, 2018 Edinburgh Scotland . I feel proud to say that I was a student of
Pakistan’s leading medical University “Dow University of Health Sciences” and now successfully
award my Medical (BDS) Degree from DUHS and continuing serve as House Officer Practices. It’s
my dream to be a well-known lady Doctor of my country and serve charity for humanity specially
people living in Pakistan. I would like to attend upcoming worldwide conferences for gain my
professional knowledge.

5) Copy of my University Education Evidences Degree & certificates / Bonafide house Officer
letter issued by “Dow University Of Health Sciences”.

6) My Original personal Bank Statement A/c __________________________sufficient Funds

available for my future education and visit expenses provided by my Father.

7) Under taking including all related documents From My loving Father
