ACCOMPLISHMENT REPORT

I. RATIONALE:
This report is all about the accomplishment from June 2016 to April 2017 of this school year. It
contains the accomplishment for the subject Earth and Life Science during first Semester (June to October,
2016) and General Chemistry 1 for the second semester (November 2016 to April, 2017) as well as all about
the activities of the whole school year including what transpired in the advisory classes HUMSS 1 (1 st Sem)
and HUMSS 5 (2nd Sem).
II. OBJECTIVES
A. TEACHING-LEARNING PROCESS
1. Prepare 80 daily lesson logs for the whole semesters.
2. Plan and vary assessments to match the different students’ need, abilities, learning style in all
classes handled within the whole year.
3. Implement/ Execute the learning competencies in the curriculum guide (Week 1 to 10 of 3 rd and
4th quarter)
B. STUDENTS’OUTCOME
1. Monitor, evaluate and maintain all student’s records in all classes throughout two semesters in a
year such as
a. SF1
b. SF2
c. SF5
d. Form -137
e. Form -138
f. On line registration in LIS
g. Class record/Grade sheets
2. Conduct remedial classes to those students who has low mastery in the learning competencies at
least twice in the whole semester.
3. Prepare at least one (1) summative exams (long exam) and two (2) quarterly exams in the whole
semester.
4. Attain a general scholastic average of 85 in all Students of STEM in chemistry.
5. Attain a general scholastic average of 80 in all Students of HUMSS and TVL 5 in Earth and Life
Science.
C. COMMUNITY INVOLVEMENT
1. Conduct at least two (2):
a. Homeroom meeting / parent-teacher conference.
b. Card Day
c. Recognition Day
2. Home visit at least two (2) students within a school year.
3. Participate in community activities such as;
a. Independence Day Parade
b. Pista Y ang Cagueban
c. Brigada Eskwela Opening
d. Rizal Day Celebration
e. Balayong Parade
D. PROFESSIONAL GROWTH AND DEVELOPMENT
1. Attend seminars, workshops and trainings.
2. Attend Department meeting, administrative meetings.
3. Participate in Gender and Development Activities.
4. Attend activities in the Teachers Day Celebration.

4. All the students of STEM attained a scholastic general average of 88 in chemistry.
5. All the students of HUMSS 1-5 and TVL 5 achieved a general scholastic average of 80 in Earth
and Life Science.
6. 0% dropped out rate for the first and second semester advisory classes.

C. COMMUNITY INVOLVEMENT
1. Conducted:
a. at least one (1) homeroom meeting during first sem/ and 1 parent-teacher conference
during second sem..
b. Card Day
c. at least two (2) Recognition Day Program (1st and 2nd Sem).
2. Home visit at least two (2) students within a school year.

4. Disseminated and Invited students to participate in the “Oplan Gupit Program” of Values Ed.
Teachers.

1. Attended Mass Training of Teachers in Senior High School last June 1 to 10, 2016. (By
Specialization) Phase 1
2. Attended Mass Training of Teachers in Senior High School last July 4-8, 2016. (Common Topics)
3. Attended Mass Training of Teachers in Senior High School last September 4-11. 2016 (By
Specialization) Phase 2
4. Attended meetings with Dr. Cyril C. Serrador on Aug. 15, 2016, October 3, 2016 and Nov. 7, 2016
as well as administrative meetings with Dr. Eduardo G. Santos on Jan 6 and Feb 28, 2017
5. Participated in Gender and Development Activities last November 23, 2016 and Dec. 9, 2016
6. Attended activities in the Teachers Day Celebration last October 7, 2016 at Robinsons Mall.
7. Served as Panelist during Pre-Oral (Jan 27-28, 2017) and Final Defense (March 24-25, 2017) of
STEM students in Research in Daily Life 1 subject.
8. Served as interviewer during mock Job Interview of HUMSS/TVL (March 3, 2017) and ABM/TVL
students last March 10, 2017 for their subject Reading and Writing Skills.
9. Prepared the General Schedule of Senior High School for the first and second Semesters of the
School year 2016-2017, as well as the General schedule for first semester in school year 2017-2018.
10. Helped the Senior High School Coordinator to check the grade sheets during third quarter second
semester.
11. Helped Dr. Carlyn Don G. Padul to encode and countercheck the awardees of GAS 4 for second
semester.
12. Prepared the Certificates of Recognition during first and second semesters as well as the
Program and invitation during First semester.
13. Served as respondent of Dr. Raul Marin in his research survey about the Senior High teachers
in STEM
14. Participated in the Optical Mission last February 24, 2017 during Foundation Day.
15. Served as panelist during proposal defense of grade 8 STE students last Mar 17, 2017 at Grade
8 Ilang Ilang Room.
16. Served as demonstration teacher during Mass training of Teachers at A and A plaza Hotel last
September 7, 2016.
17. Attended the MOA signing of Vivant, PNS and Delta P on January 31, 2017 and also prepared
and submitted list of science laboratory equipment and materials for Vivant Foundation.

IV. ANALYSIS OF THE SITUATION


This school year is very tough year because it’s the first time to offer grade 11. Difficulties in
different subject was encountered because of the different factors such as no classroom, no learning
materials and difficult subjects offered in senior high. However, these difficulties were overcome due to
constant reminder, follow up and support to the students. Despite of no classroom, no learning materials in
this school year, I know it is a fruitful year. I could say that my students learned in my class because for
instance there are times I write the name of the molecule incorrectly; they are the one correcting me. I think
they learned in my class because of the different worksheets and practice exercises that I gave them as well
as the collaborative activities they performed. Finally, more on performance task can help students
understand the lesson. I immediately remediate those students who did not understand the lesson every
friday at least twice a month that’s why they don’t failed in their chemistry subject.

V. REFLECTION
“UNDERSTAND OUR STUDENTS”, These are the lines/ words I should always remember. I
realized that students have their own trials and issues like us. The good thing is, we are adult/ matured
enough to handle those issues but the students are very young to addressed their issues. I also realized that
constant communication with our students can helped us determine who among our students need extra
effort to understand. Sometimes, we are also feeling depressed in our own lives but we need to hide this so
we can function well. I thank the Lord God I’m still surviving from my depression but I’m so sorry to those
times I feel bad in my life because of difficult situations. Moreover, I’m very proud to say that even I don’t
have class (subject handled in HUMSS 5 my advisees), all of them passed all their subjects. Although, there
are students that are high risk to stop, and other subject teacher can’t understand the situation of those
students, at the end of the day they passed all the subjects. I realized that not all teachers can give their
understanding to students because of their own issues that they don’t understand. I thank God all my students
passed their subject.
