son/daughter _______________________________________, a Grade 12 student, section
CONFIDENCE, in the Work Immersion Program as one of the course requirements for
graduation in the implemented Senior High School of K to 12 Basic Education Program under
RA 10533. I/We fully aware that he/she is physically fit to engage or participate in the Work
Immersion Program on 2nd Semester starting from October 2017 to March 2018 depending on
the given Daily Schedule of Activities by the Immersion Coordinator.
I have considered the benefits that my son or daughter will derive from his/her
participation in this activity provided that due care and precaution will be observed to ensure the
comfort and safety of my son/daughter and that Balite NHS Administrator, Teacher In-
Charge/Coordinator and the LGU may not be held responsible for any untoward incident thay
may happen beyond their control. This Parental consent is also abided by the Memorandum of
Agreement (MOA) (section IV-Liability).
