AUTHORITY
a. Section 50, Rule VIII, Implementing Rules and Regulations of RA 6975 (DILG Act of
1990) and BFP SOP Nr. IID-2006-01: Comprehensive Fire and Arson Investigation and
filing of Criminal Complaint Procedures.

MATTERS TO BE INVESTIGATED:
a. The origin and cause of the fire incident.
b. The suspect/perpetrators if there is any.
c. Others matters in the occurrence of the fire incident.

FACTS OF THE CASE

A. That on or about 2029H 16 May 2018, Radio Communication group of Mayor’s office informed
this unit about a fire incident at Brgy F. Reyes, GMA, Cavite.
B. Immediately E-221 B, E-221 C, E-221 D and E-221 E, headed by F03 Frederick T Pavo responded
to the scene and perform firefighting thereat.
C. One residential house totally gutted by fire owned by Mrs. Gerly Palawan. Estimated cost of
damage was more or less twenty thousand pesos Php 20,000.00
D. Initial investigation disclosed that the fire originated at the living room of the house of Mrs.
Gerly Palawan.
E. It is noteworthy to mention that there was no electricity on their house and they use kerosene
lamp as a source of light during the night time.
 F. It was clearly stated to the statement of Mrs. Gerly Palawan, that she left the kerosene lamp
lighted above their center table which accidentally fell on the floor

DISCUSSION

The fire occurred due to unattended kerosene lamp left on the top center table which
accidentally fell on the floor and due to flammable substance contain by the lamp, the fire
rapidly spread throughout the whole house.
