JUDICIAL AFFIDAVIT

I, GINA L. ONGPIN , 57 years of age, single, a resident of

Unit 2, #153 Brookside, Baguio City after having been duly sworn
to in accordance with law, hereby depose and state:

PRELIMINARY STATEMENT

That in accordance with A.M. No. 12-8-8-SC, which

prescribes the use of judicial affidavits to serve as the direct
examination testimony of the witness, on the basis of which the
adverse party may conduct their cross-examination on such a
witness, I hereby execute this judicial affidavit in a question and
answer format;

That conformably with section 3 (b) of the said A.M. No.

12-8-8-SC, I also state that it was SPO3 Juan Dela Cruz,
investigator of Station 7 Baguio City Police Office, who conducted
the examination of the undersigned affiant;

That conformably also with section 3 (c) thereof, I hereby

state under the pain of perjury that in answering the questions
asked of me, as appearing herein below, I am fully conscious that I
did so under oath, and that I may face criminal liabilities for false
testimony or perjury;

AND UNDER OATH, AVERS THE FOLLOWING:

1. Q : What is your occupation?

A : I am businesswoman.

2. Q : Are you the owner of Palangdao Building?

A : Yes, I am.

3. Q : Where is it located?
A : The building is located at Palangdao Building located
at Lower General Luna Road, Baguio City.

4. Q : Are there any shops in your building called Aesthetics

Beauty Shop?
A : Yes, there is.

5. Q : What specific floor it is located?

 A : The beauty shop is situated specifically at the 3rd
Floor Palangdao Building.

6. Q : Are there any business establishment similar to that

operating within the building?
A : There is none. That is the only aesthetics business in
that building.

7. Q : Who is the owner of the shop?

A : the owner is Ms. REIGN M. CANLAS.

8. Q : How sure are you?

A : Because I issued receipts under her name and the
establishment as my lessee.

9. Q : Are there any problems that arise during her lease

contract or any complaint against the shop?
A : There is none.

10. Q : Do you have anything more to add to you statements?

A : I have none for the moment.

11. Q : Are you willing to sign this affidavit consisting of three

(3) pages, to certify that all the statements you made
are true.
A : Yes sir.
