Approval Sheet

This research paper entitled MATHEMATICS ANXIETY LEVEL IN RELATION TO THE

ACADEMIC PERFORMANCE OF SENIOR HIGH SCHOOL STUDENTS OF

MINDANAO MISSION ACADEMY WITH SUBJECTS INVOLVING MATHEMATICS, by

Phoebe Jane Genovia, Alexander Gabriel Densing and Rene Tayone has been examined

by the panel members. The panel members are satisfied with the revisions made and thus

approve the proposal in its current form. Any changes between this approved version and

the thesis manuscript requires the express approval of the faculty adviser. However, should

in the course of data gathering the researchers decide to make changes to what has been

proposed, the revised paper will have to be re-defended before the panel members,

notwithstanding the adviser’s approval. In any case, the researchers must highlight the

changes between the approved and revised version on a separate sheet to be inserted in the

thesis manuscript.
