Role of head nurse

 Checks the attendance, paraphernalia, grooming and uniform of
the group member (primary student nurse)
 Makes plans of activities, special tasks and schedule lunch and
break time of unit member.
 Check the sample charting of the bedside nurse.
 Follow up the activities of the health team member, seeing to it that
all actions are coordinated to attain objectives in outmost synergy.
 Goes with the doctors round together with the primary nurse.
 Conducts pre and post conference.
 Informs the clinical instructor about any clinical procedures done
and or to be done to the patients.
 Ensure harmonious relationship of the members and to the hospital
personnel.
 Evaluates the performance of the member.
 Give supplemental care in the absence of one of the health team
members.
 Carries responsibility for any untoward incidents made by the health
team members.
 Checks the drug and know everything about the patient.
 Acts as substitute for any absences or tardiness that will be made by
any member of the team.
 Coordinates with staff activities.
 Assist the charge nurse in carrying out Doctors` order.
 Ensure that proper and complete documentation is done.
 Receives proper endorsement from the staff and primary nurse.
 Checks proper functioning of the equipment and availability of
supplies.
 Does the final rounds to the respective patient assignment (area
assignment).
 Primary nurse

 Ensure proper observance of the staff on hospital nursing service

department policies and procedures
 Knows the member assigned to specific patients in the team
 Acts as the HN in cases where SHN is absent as selected by the CI
 Receives and endorsement from the outgoing shift
 Makes the patient list and vital sign sheet (as assigned by the CI)
 Prepares minutes of pre and post conferences ( assigned by the
SHN)
 Goes with the doctors rounds
 Receives admission, endorse any special or new orders or
procedures and follow up requisition
 Monitor and record intake and output
 Assist the attending physician in the procedure
 Record IVF follow up and terminate IVF
 Maintains cleanliness and orderliness of the nurses station
 Monitors the patients status every now and then.
 Carry out Doctors order.
 Request medications from the pharmacy (GSDH encoding c/o staff)
and supplies/IVF from CSR.
 Listen and write important endorsement form outgoing shift.
 Goes with the nurse’s rounds.
 Knows the drug of the patient ahead of time/Drug study.
 Prepares and administers medications in front of the SHN while
observing 12R’s
 Prepares the meds ahead of time but with special precaution to
drugs that are to be reconstituted and kept refrigerated.
 Coordinates with the student HN before and after giving
medications.
 Makes the prescription for patients who are prescribed/excess in
PHIC/ on personal account. c/o GSDH
 Records all medications administered
 Reports any error in drug administration to the student HN
 Coordinates with the staff medication nurse before, during and after
giving of medications.
 Coordinates with staff nurse in any changes or new drug orders.
 Explains to the patient the administered drug.
 Checks the condition of the patient before and after administering
the drugs.
 Record the intake and output for those patient whose intake and
output is being monitored.
 Cleans the medication area
 Carry out going home order
 Educates the patient and significant others about the disease
process.
 Reports any unusual changes in the patient’s vital signs to charge
nurse
 Knows the IVF level of the patient and regulate it properly.
 Does the morning care of the patient.
 Follows up all IVF consumed during the shift.
 Endorse and records/graph the patient’s vital signs.
 Provides immediate care with the patient with coordination with the
staff
 Performs procedure with supervision of the staff.
 Performs nebulization to the patient’
 Sends specimen to the laboratory
 Prepares room tags.
 Do the documentation of the patient’s care
 Reports any unusualities to the patients’ condition to the student
headnurse
 Coordinates with the SHN.
NURSE’S PRAYER

Dear Jesus, model and inspiration of the nursing profession, I know

that when you are ascended into heaven, you left the care of the
sick to those of us whom you have blessed with the Holy Vocation of
nursing. Please help me to be faithful to that calling so that I can
always do the things you want me to do and in the manner that you
want me to do them. Grant that my voice be gently, that my hands
may have the softness and sympathy of your hands. That my
presence will may bring something for the hope and consolation,
which your presence brought to the sufferers of your day. I want to
do all these things Lord, but I know that I am weak and can do little
things without your aid. Please give that aid this day and every day
of my life so that I can always be what you want me to be, an angel
in the sick room. Amen.
