FAMILY NURSING HEALTH SURVEY

I. Situation of the Family

The Family Oquias is a member of Purok Sampaguita, Barangay Marcos that lives
near the shore, next to seven houses from the national road of Baybay to Ormoc. They are an
old formed family with more than a decade in formation. Their main house structure is made
of made of woods, such as plywood and bamboo. The head of the family is the only one
working as a laborer at Barangay Marcos. Their comfort room is located a meter from their
house. They do not have a living room, bedroom, kitchen and a dining area as their house is
small. It is observable that their house has inadequate space for a couple with a typical Filipino
family size. The floor in the surrounding environment is rocky and there are coconut trees
around the house.

II. The Family Nursing Assessment

A. FAMILY STRUCTURE, CHARACTERISTICS AND DYNAMICS

1. Members of the household and relationship to the head of the family


The family Oquias is a cohabiting family, comprising of two unmarried parents and 4
children. Their family authority mainly patriarchal as majority of the decisions are made by the
father. They have a gross income of Php 1,500.00 per week from the labor work of the father.
The family is located at Barangay Sampaguita, near the shore. They currently live in their own
small house made of woods, having no electricity in their home.
Table 1 presents the membership of this family.

2. Type of Family Structure (e.g. Matriarchal or Patriarchal, Nuclear or Extended, etc.)


The family is a nuclear type of family and the father is the head of the household. They
are not married. They are pure-blooded Filipino and their religious affiliation is Roman Catholic
and sometimes they cannot attend the mass every Sunday because of the time constraints
and also considering the distance from their home to the church. The Family is an egalitarian
family in which both the mother and father shared power in decision making such as money
and expenses for food and bills. For them it is important to respect their opinion and their value
perspectives for harmonious relationship and for good communication. They have equal share
of responsibilities in doing the household chores such as washing the dishes, cleaning the
house and other chores in which they participate.

3. Decision Making within the Family (dominant family member in terms of decision-
making, especially in matters of health care)
The family’s source of income is from the money that the head of the household earns
from being a laborer in which is around ₱6,000 every month. The father is responsible for
decision making in money and other expenses. He is usually the one responsible for dividing
the income for their necessities. The income money is intended and utilized for food, billings
in water and other miscellaneous things for the day to day life needs. According to the wife,
the income is insufficient because they have to pay for food for the whole family, the billing,
and loans. In terms of health care, during instances be it when someone in the family gets
sick, prenatal check-up or immunizations, both the mother and father decide for the health

care of each member of the family like going to the health center or rural health unit for
treatment and disease management.

4. General family relationship/dynamics presence of any obvious/readily observable

B. SOCIO-ECONOMIC AND CULTURAL CHARACTERISTICS

1. Income and Expenses


The head of the family is a labor worker; their gross income is Php 1,500.00 per week
from the labor work of the father. Their expenses focus more on their food and other
necessities such as soap.

2. Educational attainment of each member


The head of the family is achieved grade school level 6 and the mother achieved grade
school level 5. The couple already verbalized that they have no intention to seek higher
education after their current achievement and they had verbalized they feel they are old
already to come back to school.

3. Ethnic background and religious affiliation


The family is purely Filipino where the head of the household and the client family mother
traces their ancestry natively in the Leyte. The family can be considered Patrilocal natives
based on ancestry for the province, but in basic actuality, they are both Eastern Visayas or
Bisaya per cultural heritage.
The family does not believe on the concept of “hilot”, and quack doctor. They also do not
believe in witch craft as evil existence. The family’s religion is Roman Catholic and they do
participate in any religious activities related to their faith when not busy on other non-work and
community tasks.

4. Significant others – role(s) they play in family’s life


The mother of the client family’s mother gave them land near the shore and it is where
they currently reside, in which they built their own house. Their neighbors are also their
relatives and they use their comfort rooms for excreta purposes. Their neighbors also provide
support to them whenever the Oquias family is in need.

5. Relationship of the family to larger community – Nature and extent of participation of


the family in community activities.
The family is usually do not involve in any community activities in relation to livelihood, health
care and education. Although they showed goodness to community and give respect to
leaders. Their relationship to the BHW are good and they follow the advices made for them by
the healthcare provider.

C. HOME AND ENVIRONMENT

1. Housing
i. Adequacy of living space
The family is located at Barangay Sampaguita, near the shore. The family’s living
space is inadequate for the whole family, it is described to be as a size of a common waiting
shed. The house is only made of lumber, plywood, bamboo and iron sheets as their ceiling.
As you walk towards the house, it has poor ventilation and is slightly dark inside and you can
immediately see everything that is inside of it. Their house has no division for different sections
but they have a kitchen with open drainage, furniture like chairs and a small table, and there
are no appliances inside since there is no electricity in their home. Foul odor within the
environment is evident due to the presence of stray dogs, scattered trashes, and the open
drainage outside their house. Presence of all trees around were also observed.
ii. Sleeping arrangement
Upon assessing, the family sleeps together in their bedroom. They sleep in a small
sleeping mat which is enough for the family to be accommodated. They usually have 6-8 hours
of sleep but they wake up in the middle of the night to urinate in a communal found in their
neighbor’s house which is a safe place for them to urinate even during the night. The family
verbalized that they do not have difficulty in sleeping.
iii. Presence of breeding or resting sites of vectors of diseases (mosquitoes, roaches,
flies, rodents, etc.)
Upon assessment, it was found that there were presence of possible breeding sites of
vectors surrounding the house of the client family. Although it was noted that it may be
seasonal as when it is a rainy season there would be stagnant water in the environment of the
client family, whereas when it is a sunny day, the stagnant water would evaporate. The client
family is at higher risk of these breeding sites of vector whenever it is a rainy season.

iv. Presence of accident hazards


The housing premises itself within the family is hazardous because most of the
materials used are lightweight materials and prone to fire since they only use candle as their
light at night. There are supplies such as sharp objects and toys that are in disarray which
makes the space unfit to receive visitors and unsafe for the children when playing around.
Upon inspection of the outside part of the house, there were coconut trees around the house
and the rocky floor of the beach could be accident hazard for the family and their home.
v. Food storage and cooking facilities
The family immediately cooks and consumes any food they buy or procure such as
vegetables, and fish since the household does not have any electricity, and has no means of
preservation except for salting. The family preserves food by salting which draws water out,
preventing the bacteria from growing and spoiling the food. They store their leftover cooked
food in small bowls, with a plate serving as the cover. The dirty kitchen is located inside the
house and firewood is the primary combustion used for cooking. The smoke generated from
the firewood escapes to the outside through spaces between the wall so it does not
accumulate inside the house.
vi. Water supply – source, ownership, potability
The family’s main source of water is from Mt. Pangasugan. It is piped and connected
using a water pipe. They get water supply for their daily use through the piped system. They
also get the water which is intended for drinking through it.
They transport water on bottles which are covered. They store water through the plastic
containers which are also covered. The stored water is to be used for bathing of the spouse
and the children and other daily activities that requires washing or cleaning.
vii. Toilet facility – type, ownership, sanitary condition
Upon assessment, the family has no proper excreta disposal since they don't have
toilet of their own, whenever the need arises for excreta disposal they use their neighbors
comfort room as they are given permission by them.
Garbage/refuse disposal – type, sanitary condition
The family disposes their garbage through dumping and burning especially to plastic
materials. Their garbage is mainly composed of plastics from different grocery items, cans,
plastic bottles and leaves. It is all in the same trash bin thus it is not segregated. As their trash
bin, they use big rice sacks and medium sized drum. They clean their surrounding environment
every morning and sometimes in the afternoon. They would sometimes burn the leaves and
plastics that was swept and grouped when they clean.

viii. Drainage system – type, sanitary condition


Upon the assessment, the family have an open drainage system. The water that are
being used goes directly to the ground and into the mini canal. Their drainage system is
unsatisfactory because it is unsanitary and may cause health problems especially for the little
kids who are found in playing in the ground

D. Health status of each member

1. Medical and nursing history indicating current or past significant illnesses or beliefs
and practices conducive to health (in-depth discussion of past health history)
The client family has no past significant nor present illness. Although it can be noted that
one of the children was coughing, but it was not diagnosed and neither did the client family
find it as an illness, as verbalized by the client family mother “Na bugnawan ra siguru na siya
sir”. As for the other members the most observable health threat they had were their hygiene.
The client family mother mentioned that they would use herbs found in their surrounding
environment to treat wounds whenever their family member gets injured.

ii. Dietary history specifying quality and quantity of food/nutrient intake per day (type
of food, frequency and usually served in Family)

The client family’s mother verbalized that they usually consume rice and rootcrops as
their primary food as well as vegetables, fish, egg and meat as viand. They also consume
fruits whenever available. Their breakfast usually consists of rice, fried dried fish and egg.
Sometimes they also consume boiled vegetables also known as "law-uy" in their dialect for
breakfast. 3/4 cup of rice contains 77calories, 0.3g fats, 17.4g of Carbohydrates and 1.2g
Protein. 1 Fried fish contains 50-75 Calories, 0g Carbohydrates,5g of Fats and 7g Proteins.
1egg also contains 50-75 Calories, 0g Carbohydrates, 5g fats and 7g of Protein. 1/2 cup of
cooked vegetables contains 25 Calories, 5g Carbohydrates,0 g fats and 2g Protein. For lunch
the husband usually takes what is available during breakfast in his workplace. On the other
hand, the wife and the children consume rice and vegetables for lunch. Sometimes they eat
banana for dessert. 1banana contains 60 calories and 15g Carbohydrates. Their dinner
usually consists of rice and fish and whenever there is no available rice, they consume
rootcrops. Rootcrops such as sweet potato contains 73 Calories,17g Carbohydrates,0.1g fats
and 1.5g protein.
iv. Eating or feeding habits or practices (irregular meals)
The family usually consume their meal thrice daily. They usually consume rice, root
crops, fish, egg, vegetables and fruits. Rice and root crops are their primary source of
Carbohydrates. Fish and eggs are the sources of protein and vegetables and fruits as sources
for vitamins and other essential nutrients. The family rarely get to consume meat due to
scarcity of money since they have other priorities to spend it to. The children only get to eat
meat during feeding programs and birthday parties wherein they are invited. The couple
wanted to supply the needs of the children especially related to food and nutrition but they just
lack the resources to do so.

4. Developmental assessment of infants, toddlers, and pre-schoolers (e.g


developmental capability, psychomotor)
The client family currently have two toddlers where they manifested autonomy as they
did what they wanted to do it is as predicted in the toddler stage of Eriksonian Theory that
there is autonomy versus Doubt, and as far as observed the children were autonomous in their
actions. One child of the client family is currently on School age period where there is industry
versus inferiority, the child is not currently going to school and is not manifesting industry as
assessed. The parents based on different developmental milestones, were able to

understands and reply according to the questions being asked, thinks clearly and acts or
responds according to their age. In accordance to the Psychosexual theory of S. Freud, the
couple belongs in genital stage because they are sexual maturity and learns to establish
satisfactory relationship with her husband. In the terms of Psychosocial theory by E. Erickson
the family is on the stage of generativity vs. stagnation which this time, adults strive to crate
or nurture things that will outlast them; often by parenting children or contributing to positive
changes that benefit other people. The family also is in the phase of formal operational
thoughts as for J. Piaget cognitive development because this was the attitudes and opinions
subside. The family belongs to the postconventional because they are responsible for self-
care because they view this as a standard of adult behaviour based on the Moral Development
Theory of L. Kholberg.

5. Risk factor assessment indicating presence of major and contributing modifiable risk
factors for specific lifestyle diseases (vices and lifestyle)
The head of the family would occasionally drink alcohol as evidenced by the presence of
a few alcoholic bottles tucked inside the house. Modifiable risk factors that were observed
during assessment include improper garbage disposal practices, poor personal hygiene, and
the presence of breeding sites around the house. Moreover, the spouse is currently
unemployed and does not engage in any form of physical activity apart from the usual
household chores. However, she verbalized that she often visited her neighbors to socialize.

6. Physical assessment indicating presence of illness state/s (diagnosed or


undiagnosed by medical practitioners) (please refer to family folder in the health
center)
Upon home visit, the client family’s mother looked healthy and exhibited no sign and
symptoms of any present illness. Unfortunately, the Individual Treatment Record and the
Household Treatment Record and the Target Client List of the Brgy. Marcos Health Centre
was not reviewed for further verification of the status of Pre-Natal Check-up and Natality Plan
of the woman.

7. Results of laboratory, diagnostic and other screening procedures supportive of


assessment findings
Upon assessment the client family’s mother was not able to show any laboratory,
diagnostic and other screening procedures result.

E. Values and practices on health promotion/maintenance and disease prevention


The client family values and practices on health promotion/maintenance is limited to
traditional knowledge. They would use herbs found in the surrounding environment as
treatment for wounds. They are not aware of possible diseases that may arise from their
environment.

2. Healthy lifestyle practices


The family practiced eating healthy foods such as vegetables and fruits. They also
verbalized that they drink plenty of fluids approximately 2 litters per day to prevent dehydration.
As verbalized by the spouse they as a couple does not have any problem with themselves, in
fact for the spouse side she views herself as a strong woman since she was able to conceive
though it took them time to have conceived. She does not have any problems with any
contributing factor as to what may irritate her or trigger her mood swings.

3. Hygiene
The mother practiced poor hygiene which was illustrated through her tolerance of the
presence of dusty areas within their home, her uncut nails, unbathed children, dust in the faces
and in the arms of her children, and untidy house which needs arranging and cleaning.

4. Adequacy of:
i. Rest and sleep
The family go to sleep together at the same time from 8pm to 5am, however it is
disturbed because they drank water or have the need to urinate. The client family mother

verbalized that they don’t have trouble in sleeping and do not take sleeping medications
however the client claimed that every time there is an occasion their sleep is inadequate
especially birthdays. In afternoon the wife has taken naps in 1 to 3 hours straight.
ii. Exercise/activities
The client family mother does all the daily activities in their home while the head of the
house goes out to work. The client family mother is the one in charge in keeping their house
clean, and cooking food for the family. As verbalized by the client family mother, their typical
food is rice, fish, varieties of vegetables and fruits which is sufficient for their day to day living
iii. Use of protective measures (e.g. adequate footwear in parasite-infested areas;

The family uses mosquito net to protect them from disease carrying mosquitoes, the head of

over. They handle stress by talking first with each other and sometimes with their relatives
which is just in their neighbourhood. Way of coping stress or misunderstanding was talking
with moderation and respect to each other.

5. Use of promotive-preventive health services


The family are participative during medical mission or programs in the barangay and follow
advices from the health care provider. Whenever a client family member is sick, they always
opt for check up in the barangay health center. The client family’s mother verbalized that in
order to be healthy they must also eat foods that promote health.

V. Recommendation/Suggestions
Recommendations
Based on the findings, the student nurses concluded that the factors that lead to the
development of the problems were age, environment and her sedentary lifestyle.
We would like to recommend the following:

To the Oquias Family


 To give more attention to their personal hygiene and ensure that each member is able
to practice the proper hand washing technique
 To segregate and throw their garbage in the proper containers
 To observe for any potential breeding sites around the household and perform the 5S-
Search and destroy breeding places, seek early consultation, secure self-protection
measures, say no to indiscriminate fogging, and to sustain hydration.
 To remain alert for the potential accidental hazards mentioned in this study, namely:
falling coconuts and rocky floor. It is recommended that the family take the appropriate
action in order to mitigate the harms caused by the said hazards
 To cover their mouths when sneezing or coughing so as to prevent transmission of
pathogens during sleeping hours when the family is huddled together as they sleep.

 To have an open mind and maintain sensitivity towards the situation of the client family

 To become aware of the difficulties that they may encounter in making the said case,
such as limited interaction time with the patient and the patient’s compliance to nursing
activities and gathering of data, so as to make the necessary preparations before the
actual interaction between student nurse and client is made.
 To be equipped with the proper and sufficient knowledge necessary for the well-being
of the family and community. The student nurse should know the principles of asepsis
and ways to reduce contamination, as well as the proper garbage disposal methods,
the 5S, and any principle essential to the community.
 To gain an understanding of the client family’s situation and refrain from judgement,
so as to provide them with the necessary care and interventions needed for the well-
being and development of the family.
 To be able to communicate well with the client. The student nurse must be competent
in communicating with the client and must refrain from using any medical jargon so as
to establish a good initial relationship with the client family.

To Clinical Instructor
 For the clinical instructors to be able to assist the student-nurses in rendering the
appropriate interventions in meeting the needs of the client
 For the clinical instructors to be able to teach the student-nurses in the correct way to
perform Leopold’s Maneuver so that they could check the fetal position and fetal heart
rate of the pregnant women in the community.
 The should train student-nurses on how to obtain anthropometric measurements and
compute for the Body Mass Index in order to prevent obesity or malnutrition in the
community and for them to give health education about proper nutrition.
 They should train the student-nurses in providing holistic care to the people in the
community.

To Health Care Team


 The Health Care Team should conduct seminars about proper waste segregation,
environmental safety, prevention from communicable diseases, diseases acquired
from vectors and other possible problems that might arise in the community.
