INTRODUCTION

Community health nursing is a unique blend of nursing, public health

practice, and other types of social assistance designed as a human service for

humankind's development. This form of nursing is holistic in the sense that it

encompasses all aspects of health needs, but since it focuses on the community,

it explores the heavier side of the spectrum of health needs, which increases the

physical, mental, and emotional demands of nursing.

Therewith, community health nursing has the potential to instigate beneficial

changes in the community, given that it requires a deep connection with the entire

community that allows for the creation of a collective community diagnosis.

Although different from the usual diagnosis encountered in the school and hospital,

community diagnosis shares the same nature as common diagnoses that aim to

deliver care to people who are less fortunate and are dependent on accessible

healthcare. This form of nursing is especially effective for people who live away

from the city where most healthcare services are easily available. It is also

significantly cost-effective and gives the clients freedom to continue their routines

while receiving the care that they need.

GENERAL OBJECTIVE

After a week of community health nursing duty in Purok Lapu-Lapu,

Guadalupe, Maasin City, the student nurses would be able to promote health and

prevent diseases by performing nursing interventions that promote consciousness,

awareness, and recognition of the importance of health, proper sanitation, and

good personal hygiene, and how the absence of these factors can lead to

consequences that affect their daily living.

SPECIFIC OBJECTIVES

1. Establish rapport with families in the community through the interviews

conducted.

2. Determine problems present within every family interviewed, as well as the

entire community.

3. Identify the priority problems for each family.

4. Determine the most appropriate nursing interventions for each problem of

the families, as well as the diagnosis for the community.

5. Provide health education to the priority family and ensure that they learn

everything for them to apply the knowledge in their everyday lives.

6. Discuss the different diseases that the priority family is most prone to, as

well as the preventive measures that they can do in order not to contract

any of the diseases.

BARANGAY HISTORY

Barangay Guadalupe is celebrating barangay day every 22nd day of June

ten years ago with no basis for holding on to such a particular date.

As told by the earlier inhabitants of the place, the late Severo Galeon, 100

years old & the late Precioso Maturan, 95 years old and handed down from me

generation to the next that the barangay day was founded on December 9 and it

was the first celebration done by the late Rufino Aroy Kirong, the first Kapitan del

Barrio.

BARANGAY PROFILE

a. Geographical Location

Barangay Guadalupe is one of the 70 barangays in the City of Maasin, in

the province of Southern Leyte, which is located North of Barangay Bilibol, East of

Tawid and West of Barangay Cabulihan and Barangay Sto. Rosario. It has a total

land area of 150 hectares with the distance from the Poblacion of 16 kilometers.

Thirty (30) hecatares of the land was used for agricultural activities, one hundred

three (103) hectares for residentials and ten (10) hectares for institutional, and

seven (7) hectares for other purposes.

b. Physical Description

The household population of Barangay Guadalupe for 2018 based on the

profiles gathered at Maasin City Planning and Development Office is 334, having

a population of 1,506. The Barangay has eight (8) puroks namely: Purok Tulingan,

Purok Dilis, Purok Bangus, Purok Tangigue, Purok Maya-maya, Purok Lapu-Lapu,

Purok Humayan, and Purok Kamohoy. It has Elementary and High School.

Barangay Guadalupe is empowered and designed to actively cooperate

the dedication of its barangay officials in the discharge of their duties and function.

We envision Barangay Guadalupe as the nautical port of Maasin City with

active, self-reliant, disciplined, and God-loving constituents living in a clean,

The graph shows that out of 67 families surveyed in Purok, Lapu-Lapu,

Guadalupe, and Maasin City according to their age group. With a total of 271

members in the family, there are 15 who are 0-6 years old with5.53 %, followed by

33 of them are 7-15 years old with 12.17%, 40 of them are 16-25 years old

with14.76%,38of them are 26-35 with 14.02%, 45 of them are 36-45 years old with

16.60%, 32 of them are 46-55 years old with 11.80%, 42 of them are 56-65years

old with 15.49%,15 of them are 66-75 years old with 5.53%, and 11 of them are

76-90ears old 4.05%.

It shows in the chart the hat majority of the Population is between the ages of 36-

45 years old based on 67 households that we interviewed in Purok, Lapu-Lapu,

Guadalupe, and Maasin City.

Based on the 2020 Census of Population and Housing (2020 CPH), the

Philippines had a total population of 109,035,343 persons. Of this total, the

household population comprised 99.7 percent or 108,667,043 persons. By age

group, 33.4 million (30.7%) were under 15 years of age (young dependents). On

the other hand, persons aged 15 to 64 years (working-age or economically-active

population) totaled to 69.40 million (63.9%) while those in age groups 65 years

and over (old dependents) comprised the remaining 5.86 million (5.4%). In 2015,

persons aged 0 to 14 years, 15 to 64 years, and 65 years and over accounted for

32.0, 63.3, and 4.8 percent, respectively, of the household population. Moreover,

there were more males (88.8%) than females (86.6%) among the 0 to 54 age

group. Meanwhile, among the older age group (55 years and over), females

(13.4%) outnumbered the males (11.2%). The same trend was also observed in

The pie chart illustrates the civil status of each family member, with a total

of 271 people in the 67 households that were interviewed. According to the

statistics acquired, 51.66% of people are single (140), followed by 42.80% who are

married (116), 5.16% who are widowed (14), and 0.36% who are separated (1).

The data gathered implies that the majority of 271 people in the 67

households are Single. This is due to most of them still studying and getting

married is not their top priority at the moment, also some of them tend not to get

married even though they are at the right age to enter into a lifetime relationship.

PSA in a February 2020 report said that there are 34.8 million singles or unmarried

individuals which accounts for 44% of the total population ten years old and above.

The census of population is conducted by the agency every ten years. It collects

data on the marital status of persons 10 years old and over. The PSA said that of

the total single population, males account for 54.1% which results in a ratio of 118

males for every 100 females. It added that single is defined as someone who has

never been married to anyone in their lifetime, citing the 2015 Census of

Population.

Educational Attainment

The educational attainment of the people living in Purok Lapu-Lapu in

Barangay Guadalupe Can-iwan, is depicted in this graph. It reveals that nearly

30% of Can-iwan residents hold college degrees, about 16% did not complete high

school or are still enrolled in high school, nearly 14% have completed high school,

12% are currently enrolled in college or have stopped their college studies, about

11% are in elementary school or have stopped their elementary studies, nearly 9%

are children or were not identified, about 8% have completed grade school, and

1% have completed ALS.

Education contributes to the achievement of all these goals within a

community while also contributing to individual economic growth and social

stability. Education reduces poverty by providing knowledge and skills that enable

people to earn more money; it generates income thru increased productivity,

participation in the labor force, and entrepreneurship. Education decreases

environmental degradation by teaching people about sustainable practices such

as recycling and conservation; reducing poverty, one of the biggest causes of

global deforestation; and providing people with tools to avoid overexploiting natural

resources such as forests for shelter or fuel.

The ratio of men to women living in Purok Lapu-Lapu is depicted in this

graph. According to this information, men make up the majority of the purok Lapu-

Lapu population. Males make up 52.55% and females make up 47.45%.

Having more males in a community than females affect a community’s

livelihood. Recent research illustrates that many initiatives intended to improve

livelihoods still proceed as “gender blind,” failing to account for the norms and

relations that will influence how women and men experience opportunities and

outcomes. It was found that men can pursue a broader range of livelihood activities

than women who tend to be constrained by individual perceptions of risk and

socially prescribed physical mobility restraints.

Religion

Roman Catholic Fundamental Baptist Church Jehova's Witness None

As for the religion of the people part of the Can-Iwan community, the

majority is Roman Catholic, only a few are not. This chart shows that 96.15% of

the community population are Roman Catholic, 1.75% don’t have any religion,

0.70% are Fundamental Baptist Church, and 1.40% are Jehova’s Witness.

The result of the chart shows that the community almost has the same

beliefs as each other because of their ancestors who passed it down generation

by generation. Meanwhile, there are some people who changed their beliefs

because of what they have discovered throughout their existence.

According to Jack Miller in Asia Society, The Philippines proudly boasts to

be the only Christian nation in Asia. More than 86 percent of the population is

Roman Catholic, 6 percent belong to various nationalized Christian cults, and

another 2 percent belong to well over 100 Protestant denominations.

For the occupation rate for Purok Lapu-Lapu in Can-Iwan, most residents

out there are unemployed. Some are retired because of having low capabilities to

do the job, or getting older rather. Some are just housewives, and some are still

students. But in some cases, there are students who are already working because

of their socioeconomic status. There are also some members of the family, mostly

fathers, take two jobs so that their family can survive for their everyday living. Some

members of the family also have professional jobs and works in a company.

According to The Pinoy OFW, the Philippines’ high unemployment rates are

partially attributable to the country’s subpar educational system. There are several

variables that contribute to this issue, some of which include the low quality of

teachers, a high dropout rate, a lack of competitiveness and relevance, limited

access to information technology resources, and inadequate school infrastructure.

The graph shows that the majority of the households in Lapu-Lapu have

either a nuclear or multigenerational structure. The Filipino family is considered a

highly valuable social institution in the Philippines. In general, Filipino families are

bilaterally extended and residentially nuclear.

This is a common result for Filipino households as we all value family

orientation over anything else. Moreover, the minorities show that some people in

Lapu-Lapu are either alone or have basically no children.

The graph shows that majority of the households have the mother as the

first person to decide what to do for every family matter. This destroys the

patriarchal structure of the Filipino household, although it is not certain that all of

these households have masculine heads who preside over the family members,

automatically making the mother the head of the household.

Research findings, however, suggest that Filipino families may be more

egalitarian and less patriarchal. Although fathers continue to be identified as the

"breadwinners” or the "household heads" of families, the significant roles of

mothers in family survival and decision-making are well-documented. Studies in

the 1960s and the early 1970s show that joint husband-wife decision-making

patterns are the common norm in Filipino homes (Guerrero 1965; Porio, et al.

1975; Castillo 1976). This conjugal mode of decision-making has been consistently

supported by even more recent studies conducted in the last decade (Alcid 1986;

As for the minority that is the eldest son deciding on family matters, it is

either that they live with their siblings or they are the breadwinner of the family.

When it comes to the matter of health, the majority of the households in

Lapu-Lapu have mothers or to the same extent, both parents or other people such

as health professionals decide on it. The minorities have their fathers and eldest

children decide on it.

In the Philippines, the family acts as a caregiver, patient advocate, and

primary decision-maker on matters related to the care of those dying or seriously

The Merriam-Webster online dictionary defines perceived as "to regard as

such." In order to improve health outcomes, it is critical in this study to understand

community members' perceived needs. Perceived need refers to an individual's

own judgment about the necessity or benefits of a particular service (Jiska Cohen-

Mansfield, August 2008). According to water.org, more than 3 million Filipinos rely

on unsafe and unsustainable water sources, and 7 million do not have access to

improved sanitation. The country is rapidly urbanizing, and its expanding cities are

struggling to provide adequate water and sanitation services to new residents.

Proper healthcare may be a common need in the realm of Public Health, but how

these problems are handled in a rural setting versus an urban setting is vastly

different. People who perceive barriers try coping strategies such as delaying

treatment and self-care, which can be harmful.

qualities of a good leader they want are 82% an active leader in the community,

22.3% a responsive and approachable leader, 16% responsible leader, 12% with

good attitude leader, 9% open minded leader, 6% a committed leader, and 1.4%

unbiased/educated/religious.

The data result clearly says that Purok Lapu-lapu wants an active leader.

By the fact that an active leader is an open and honest communicator committed

have received benefits and services from the following Non-Government

Agencies: Red Cross (6%), UNICEF (6%), Purefoods (1%), SLEMCOOP (1%),

Livelihood Organization (1%), BEC (1%), DUNGANON (3%). The remaining

responses have stated that they have not received an benefits or services from

Non-Government Agencies, which amounts to 81%.

This indicates that the programs from Non-Government Organizations have

reached and have benefited to only less than half of the 67 households surveyed

in Purok Lapu-Lapu, Guadalupe, Maasin City.
