Etiology
The place had no known particular name except that it was the place for
gathering pieces of wood for building materials and kindling. Hence, some opined that it
came from the word “Kalap” which means to gather logs.” Another theory holds that
Calapan was originally pronounced as “Kalapang” which, according to an old Tagalog
dictionary, was a synonym for “sanga” or branch. It could then refer to the settlement of
Kalapang as a branch of its mother town of Baco, an adjoining town. Eventually,
through time, the verb became a noun, to point to these pieces of wood and with the
suffix – an, it became Kalapan, a place where such wood was gathered. With the
Spanish influence, it has been called Calapan ever since.

What is Planning?
The plan for nursing action or care is based on determined actual or potential problems
that are identified and prioritized.
44 | P a g e
The phase is a process which primarily concerned with choosing from among possible
courses of action. Selecting appropriate types of nursing intervention, identifying right
and available resources for care and developing an operational plan and selected plan
of action is based on those things that the family could not perform themselves. Those
that facilitate action that remove barriers to care and those that improve the capacity of
the family and to make them stand in their behalf.
The planning stage is where goals and outcomes are formulated that directly
impact patient care based on EDP guidelines. These patient-specific goals and the
attainment of such assist in ensuring a positive outcome. Nursing care plans are
essential in this phase of goal setting. Care plans provide a course of direction for
personalized care tailored to an individual’s unique needs. Overall condition and
comorbid conditions play a role in the construction of a care plan. Care plans enhance
communication, documentation, reimbursement, and continuity of care across the
healthcare continuum.
This is a form of education used by the community health nurses which assist the
community to bring out about changes in lifestyle which will boost their optimum level of
functioning. This teaching and learning process covers the two major health task and
these are health promotion and prevention of illness where in the community will
improve and further develop their sense of self-reliance that allows them to prosper and
maintain a good healthy way of living.
Teaching plan should include what will be taught, when teaching will occur,
where teaching will take place, who will teach and learn, and how teaching will occur.
Deciding what will be taught is a decision you and the patient need to make together.
45 | P a g e
Although you begin as the content expert, your goal is to make your patient as
competent as he needs to be to manage his or her own health care needs.
