    rupture blood vessel in the brain resulting in the loss of consciousness for almost 8 hours ,
    paralysis and damage to speech.

    Ms. Saguid is Barangay Health Worker of Maligaya, Gloria Oriental Mindoro since February
    1990 until present. She lead the federation and hold significant position up to regional level.
    With this, I hereby endorsed that she is a rigthful beneficiary to claim the said benefits.
