May we request for the approval of travel order of Dr. Grazelle Anne F. Carido, MPH, Medical
Officer – IV and Margelyn C. Palaran, Administrative Aide III, to attend a 3-days Infection
Prevention and Control Training on May 23 to 25, 2023 at Hotel Guillermo in Pagadian City,
Zamboanga del Sur on Official Business. Transportation and per diems shall be chargeable against
Olutanga Municipal Hospital.
