We, the Grade 12 HUMSS-2 Students of San Beda College – Mendiola, are conducting a
research entitled “The Effects of Classroom Environment to their Academic Performance of
the Senior High School Students of San Beda College – Mendiola”. We are inviting the
students to participate in our research study by completing the attached surveys.

By answering the survey questionnaire, they are to follow and read the directions carefully and
answer all questions as honestly as possible. We will also assure that the answers on the
questionnaire will serve as confidential. To assure confidentiality, the answers to the
questionnaire will be recorded and for our group only. Their participation will be likely helpful
to the researchers. We will also ask permission to get their academic grade, this will be also
confidential.
